摘　　要：目的探讨微创经皮锁定钢板内固定(MIPPO)技术治疗胫骨远段骨折的临床疗效。方法采用MIPPO技术治疗37例胫骨远段骨折患者。记录手术时间、术中出血量、骨折愈合时间及并发症发生情况,比较手术前后膝、踝关节活动度及Olerud-Molander评分。结果患者均获得随访,时间6~18个月。手术时间45~75(64.4±5.9)min,术中出血量30~95(74.7±9.5)ml,骨折愈合时间6~14个月。未发生切口感染、皮肤坏死、关节活动僵硬等并发症。末次随访时,关节活动度:膝关节屈曲125°~135°(128.0°±6.0°),踝关节背伸20°~40°(29.5°±4.2°)、跖屈20°~40°(35.6°±5.3°);Olerud-Molander评分40~70(65.9±6.2)分。结论MIPPO技术治疗胫骨远段骨折具有创伤小、术后并发症少、患者功能恢复好等优点,疗效满意。Objective To investigate the clinical efficacy of minimally invasive percutaneous locking plate osteosynthesis(MIPPO)technique in the treatment of distal tibial fracture.Methods A total of 37 patients with distal tibial fractures were treated with MIPPO technique.The operation time,intraoperative blood loss,fracture healing time and complications incidence were recorded.The range of motion of the knee and ankle,Olerud-Molander score were compared before and after operation.Results All patients were followed up for 6~18 months.The operative time was 45~75(64.4±5.9)min,the intraoperative blood loss was 30~95(74.7±9.5)ml,and the fracture healing time was 6~14 months.No complications such as incision infection,skin necrosis,joint stiffness occurred.At the last follow-up,the joint range of motion:knee flexion was 125°~135°(128.0°±6.0°),for the ankle joint,dorsiflexsion was 20°~40°(29.5°±4.2°),and plantar flexion was 20°~40°(35.6°±5.3°).Olerud-Molander scores were 40~70(65.9±6.2)points.Conclusions MIPPO technique in the treatment of distal tibial fracture has the advantages of less trauma,fewer postoperative complications and good functional recovery.The curative effect is satisfactory.
