补肾活血法治疗帕金森病的用药规律研究

摘　　要：目的 基于中医传承计算平台研究补肾活血法治疗帕金森病的临床用药规律。方法 选择2009年1月至2020年12月我院中医科运用补肾活血法治疗帕金森病门诊处方,共收集获得处方591首,运用中医传承计算平台分析其用药规律。结果 591首处方涉及中药225味,功效以补虚类、活血化瘀类和清热类药物为主;四气以温、寒、平为主;五味以苦、甘、辛为主;归经以肾经、肝经、心经、脾经为主。用药频数≥120次的中药有20味,常用药对有“肉苁蓉-当归”、“丹参-川芎”等。补肾活血法临床治疗帕金森病的基本方组成为肉苁蓉、山茱萸、知母、川芎、石菖蒲等。聚类分析发现,五组核心药物组合分别发挥益气填精、滋阴清热、活血通络、豁痰开窍、通腑泄热之功效。结论 补肾活血法治疗帕金森病的常用中药、药对和基础方加深了对补肾活血法临床用药体系的认识。Objective To study the clinical medication rules of Bushen Huoxue in the treatment of Parkinson’s disease(PD) based on Traditional Chinese Medicine Inheritance Computing System(TCMICS).Methods The prescriptions for PD treated by Bushen Huoxue in the TCM department of our hospital from January 2009 to December 2020 were collected.TCMICS was used to analyse the clinical medication rules among the 591 collected prescriptions.Results There were 225 Chinese medicines out of the 591 prescriptions.These medicines mainly belonged to deficiency tonifying, blood activating and stasis removing, and heat clearing.Main four qi of TCM were warm, cold and calm;The five flavors were mainly bitter, sweet and spicy;The meridians mostly belonged to kidney, liver, heart and spleen meridians.There were 20 drugs with a frequency of medication ≥120.And the most common drug pairs were Cistanche-Angelica, Salvia miltiorrhiza-Chuanxiong, and so on.Correlation rule analysis showd that the basic prescriptions of Bushen Huoxue consisted of Cistanche,Cornus,Anemarrhena,Chuanxiong,Shichangpu,etc.Cluster analysis indicated the five core drug combinations exerted the effects of replenishing qi and essence, nourishing yin and clear heat, promoting blood circulation and unblocking collaterals, eliminating phlegm and opening the mind, and dredging the fu organs and relieving heat.Conclusion We summarize the commonly used drugs, drug pairs and basic prescriptions, and five types of prescriptions of five different clinical characteristics(deficiency, heat, blood stasis, phlegm and secretion) for PD treatment, forward to an objective and systematic understanding for clinical medication rules for the Bushen Huoxue treatment of PD.
