摘 要:骺板是儿童生长发育过程中特有的重要生理结构,同时也是最为脆弱的部分,可由骨折、感染、缺血、肿瘤、辐射、热损伤、药物、遗传及其他医源性损伤等形成骨桥。临床上骨桥的形成可导致患儿肢体的短缩及成角畸形,严重影响患儿的肢体功能。随着研究的不断深入,其发病机制涉及骨骺及干骺端血管生成,同时相关炎症介质及血管内皮生长因子(VEGF)的表达,促进了骨桥的形成。本研究对骺板这一特殊生理结构进行详细剖析,从而归纳总结骨桥形成的机制,损伤治疗中存在的风险因素,以及骨桥的诊断治疗策略。旨在为今后临床诊治儿童骺板骨桥过程中,能够提前规避风险,预判相关治疗结局,制订个性化治疗方案,从而缓解患儿病痛。Epiphyseal plate is a unique and important physiological structure in the process of children’s growth and development,and it is also the most vulnerable part.Bone bridge can be formed by fracture,infection,ischemia,tumor,radiation,heat injury,medicine,heredity,and other iatrogenic injuries.The formation of bone bridge in clinic can lead to shortening and angular deformity of children’s limbs,which seriously impacts children’s limb function.With the continuously deepening of researches,it is found that its pathogenesis involves the epiphyseal and metaphyseal angiogenesis,and is simultaneously impacted by the expressions of related inflammatory mediators and vascular endothelial growth factors(VEGF),which promotes the formation of bone bridge.In this paper,the epiphyseal plate,a special physiological structure,is analyzed in detail,so as to generalize and summarize the formation mechanism of bone bridge,the risk factors existing in injury treatment,and the diagnosis and treatment strategies of bone bridge.The aim is to avoid risks in advance,predict the related treatment outcome and formulate personalized treatment plan for the future clinical diagnosis and treatment of children’s epiphyseal plate bone bridge,so as to reduce the pain of children.
