摘　　要：目的:了解嘉定区老年严重精神障碍患者医养服务需求,分析相关影响因素,为制定可行、有效的老年严重精神障碍患者医养服务政策提供基础依据。方法:于2021年采用自制调查问卷对分层整群抽样后选取的患者进行问卷调查,共发放调查问卷267份,有效回收252份,问卷有效回收率(94.38%)。采用单因素方差分析和多重线性回归分析对影响老年严重精神障碍患者医养服务需求的相关影响因素进行分析。结果:受调查者服务需求总评分为(15.611±9.105)分,总体需求程度较高。对服务项目有需求的前10项是全科医生定期访视222例(88.10%)、精神疾病评估182例(72.22%)、代为配药服务176例(69.84%)、躯体疾病的诊治与护理174例(69.05%)、精神科医生定期访视169例(67.06%)、生活照料服务164例(65.08%)、药物管理152例(60.32%)、提供膳食151例(59.92%)、精神科药物治疗149例(59.13%)、住院送医服务146例(57.94%)。单因素分析结果显示,不同文化程度、婚姻状况、就业情况、子女关系、目前养老方式可能与老年严重精神障碍患者医养服务需求存在相关性(P<0.05);多重线性回归分析结果显示,文化程度、就业情况、养老服务方式是老年严重精神障碍患者医养服务需求的影响因素(P<0.05)。结论:对老年严重精神障碍患者,需要融入医疗护理保健等相关服务以丰富完善养老保障体系。患者的养老方式、文化程度、就业情况等值得在未来提供服务的过程中予以关注。Objective To understand the medical and nursing service needs of elderly patients with severe mental disorders in Jiading District,analyze the relevant influencing factors,and to provide rationale for formulating feasible and effective policies.Methods In 2021,a self-made questionnaire was used to conduct a questionnaire survey on the patients selected after stratified cluster sampling,a total of 267 questionnaires were distributed,252 were effectively recovered,and the effective recovery rate of the questionnaire(94.38%).Single-factor analysis of variance and multiple linear regression analysis were used to analyze the relevant influencing factors that affect the medical care service needs of elderly patients with severe mental disorders.Results The total score of service demand was(15.611±9.105)points.The top 10 service items on demand are as follows:222 persons(88.10%)in regular visits by general practitioners.182 persons(72.22%)in psychiatric assessment,176 persons(69.84%)in dispensing service on behalf,174 persons(69.05%)in treatment and nursing of physical diseases,169 persons(67.06%)in regular visits by psychiatrists,164 persons(65.08%)in life-care services,152 persons(60.32%)in drug administration,151 persons(59.92%)in providing diet,149 persons(59.13%)in psychiatric medicine treatment and 146 persons(57.94%)in hospitalization.Single factor analysis showed that different education levels,marital status,employment status,child relationship,and current care methods were related factors that affected the demand for medical care services for elderly patients with severe mental disorders(P<0.05);The results of multiple linear regression analysis showed that the model’s educational level,employment status,and service methods included in model had statistically significant effects on the needs of elderly patients with severe mental disorders(P<0.05).Conclusions In the process of providing for the aged,it is necessary to integrate medical care and other related services to enrich and to improve the old-age sec
