小横切口疝囊超高位结扎与单孔腹腔镜下疝囊高位结扎治疗小儿斜疝的效果比较

摘　　要：目的:比较小横切口疝囊超高位结扎术与单孔腹腔镜下疝囊高位结扎术治疗小儿斜疝的效果。方法:选取武进人民医院2017年1月—2020年12月收治的100例小儿斜疝患儿,根据手术方法不同分成对照组与观察组各50例。对照组采用小横切口疝囊超高位结扎术,观察组采用小儿单孔腹腔镜下疝囊高位结扎术。对比两组手术相关指标及术后并发症,术后随访3个月、6个月、1年,观察对侧隐匿疝及复发情况。结果:观察组手术时间及出血量少于对照组,但住院费高于对照组,差异有统计学意义(P<0.05)。观察组术后疼痛程度和并发症发生率低于对照组,差异有统计学意义(P<0.05)。随访1年两组患儿均无复发。对照组无对侧疝发生,观察组术中发现6例有对侧隐匿疝的患儿,且同时治疗。结论:相较于小横切口疝囊超高位结扎,给予小儿斜疝单孔腹腔镜疝囊高位结扎术治疗优势明显,安全性高,切口隐蔽,能减轻患儿痛苦,但费用较高。Objective To study small transverse hernia sac super high ligation and single hole laparoscopic hernia sac high pediatric oblique hernia treatment.Method Randomly selected 100 children oblique hernia treated from Jan.2017 to Dec.2020,divided into control group(using small transverse hernia sac super high ligation,50 cases)and observation group(50 cases,using children single-hole laparoscopic hernia sac high ligation),observe and compare surgery related indicators and postoperative complications,postoperative follow-up 3 months,6 months,1 year,observe bilateral occult hernia and recurrence.Results The operation time and blood loss in the observation group were less than those in the control group,but the hospitalization cost was higher than that in the control group,and the difference was statistically significant(P<0.05).The postoperative pain degree in the observation group was significantly lower than that in the control group(P<0.05).There was no recurrence in either group after 1 year of follow-up.There was no contralateral hernia in the control group,and 6 children with contralateral occult hernia were found in the observation group and treated at the same time.Conclusion The high ligation of hernia sac under single hole laparoscopy in children has obvious advantages of high ligation,low damage,reduce pain,high safety and concealed incision.
