甘麦大枣汤联合益坤饮治疗更年期抑郁患者激素水平、负面情绪及临床症状的效果

摘　　要：目的探讨更年期抑郁患者临床治疗中同时服用甘麦大枣汤和益坤饮的效果。方法从河源市中医院2020年4月至2021年5月收治的更年期抑郁患者中选取78例,通过随机数表法分为两组。对照组(39例)患者口服益坤饮汤剂进行治疗,观察组(39例)患者同时口服益坤饮汤剂和甘麦大枣汤进行治疗。观察两组服药治疗2个月后抑郁[抑郁自评量表(SDS)评分]、更年期症状(Kupperman评分)、激素水平[促黄体生成素(LH)、雌二醇(E2)等]及睡眠质量[总睡眠时间(TST)、睡眠潜伏期(SL)等]的变化情况。结果治疗后观察组SDS及Kupperman评分均低于对照组,差异有统计学意义(t=4.279、11.503,均P<0.05);观察组治疗后FSH、LH水平均低于对照组,E2水平高于对照组,差异有统计学意义(t=8.264、8.671、3.257,均P<0.05);治疗后观察组TST、SE大于对照组,SL、AT、AWT则小于对照组,差异有统计学意义(t=13.068、8.322、27.074、7.894、24.417,均P<0.05)。结论更年期抑郁临床治疗中同时服用益坤饮、甘麦大枣汤能够明显提高患者抑郁、更年期症状及睡眠障碍的改善效果,促进激素水平获得有效调节。Objective To investigate the clinical effects of Ganmai Dazao Decoction plus Yikun Decoction on menopausal depressed patients.Methods A total of 78 menopausal depressed patients admitted to Heyuan hospital of Traditional Chinese Medicine from April 2020 to May 2021 were selected and divided into a control group(n=39)and an observation group(n=39)according to the random number table method.The patients in the control group were treated with oral Yikun Decoction,while the patients in the observation group were treated with oral Yikun Decoction plus Ganmai Dazao Decoction.The changes in depression(self-rating depression scale[SDS]score),menopausal symptoms(Kupperman score),hormone levels(luteinizing hormone[LH],estradiol[E2],etc.)and sleep quality(total sleep time[TST],sleep latency[SL],etc.)in both groups were observed after 2 months of treatment.Results After treatment,the SDS and Kupperman scores in the observation group were lower than those in the control group,with statistically significant differences(t=4.279,11.503,both P<0.05).Meanwhile,the follicular stimulating hormone(FSH)and LH levels were lower,while the E2levels were higher in the observation group than in the control group,with statistically significant differences(t=8.264,8.671,3.257,all P<0.05).In addition,TST and sleep efficiency(SE)were longer/higher,while SL,AT and actual awake time(AWT)were shorter in the observation group than in the control group,with statistically significant differences(t=13.068,8.322,27.074,7.894,24.417,all P<0.05).Conclusion The application of Yikun Decoction plus Ganmai Dazao Decoction in clinic can significantly improve depression,menopausal symptoms and sleep disorders,and promote the effective regulation of hormone levels of menopausal depressed patients.
