摘　　要：腹泻型肠易激综合征(IBS-D)是临床常见的功能性肠病,传统医学认为其关键病机为肝郁脾虚,杨小军教授认为脾虚为本病的病机基础,肝郁为病程迁延反复的关键因素,常兼夹痰湿、湿热、气滞等,治疗上系统运用李中梓在《医宗必读》中提出“淡渗、清凉、升提、甘缓、酸收、疏利、温肾、燥脾、固涩”的“治泻九法”以标本兼治,其中又尤为推崇“疏利法”之中的疏肝法以理气解郁、调畅情志,同时结合因地、因时、因人制宜的思想,灵活遣方用药,多能达到理想的治疗效果。Diarrhea predominant irritable bowel syndrome(IBS-D)is a common functional bowel disease in clinical practice.From the traditional medicine perspective,the key pathogenesis of IBS-D is liver depression and spleen deficiency.Professor Yang Xiaojun,the tutor,believes that spleen deficiency is the pathogenesis basis of the disease,and liver depression is the key factor for the prolonged course of the disease and the recurrence.The disease is often accompanied with phlegm dampness,damp-heat,qi stagnation,etc.In terms of treatment,the"Nine Methods of Treating Diarrhea"that Li Zhongzi proposed in the Essential Readings from the Medical Tradition(Yi Zong Bi Du)can be systematically applied to treat both the symptoms and the root causes,i.e.,"draining dampness,clearing,ascending,relaxing by the sweet,astringing by the sour,dredging,warming the kidney,drying the spleen,and controlling".In particular,Li Zhongzi highly praised the liver soothing method in the"dredging"method to regulate qi,relieve depression,and regulate emotions.At the same time,combined with the thought of adjusting measures to differing conditions in terms of locality,time and persons involved,flexibly prescribing drugs can possibly achieve ideal therapeutic effects.
