养阴清肺汤治疗阴虚热毒型原发性干燥综合征的临床观察

摘　　要：目的观察养阴清肺汤治疗原发性干燥综合征的临床效果。方法选取2019年1月至2022年1月于临沂市中医医院就诊的70例原发性干燥综合征患者,采用随机数表法分为观察组和对照组,每组各35例。对照组予艾拉莫德治疗,观察组在对照组的基础上加用中药口服,连续治疗12周,比较两组患者的临床疗效、ESSDAI评分、ESSPRI评分、双眼Schirmer’s试验、唾液流率及免疫球蛋白G(IgG)、白介素-6(IL-6)、白介素-17(IL-17)水平。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后两组的ESSDAI评分、ESSPRI评分、双眼Schirmer’s试验及IgG、IL-6、IL-17水平均较治疗前明显改善,且观察组优于对照组,差异有统计学意义(P<0.05)。治疗后观察组唾液流率较治疗前改善明显,差异有统计学意义(P<0.05),对照组唾液流率较治疗前无明显改善(P>0.05)。结论中药养阴清肺汤治疗干燥综合征能显著提高患者的临床疗效,改善患者临床症状及炎症指标,促进唾液腺与泪腺功能的恢复,同时能够调节体内异常免疫反应。Objective To observe the clinical efficacy of Yangyin Qingfei Decoction in the treatment of primary Sjogren’s syndrome.Methods A total of 70 patients with primary Sjogren’s syndrome admitted to Linyi Traditional Chinese Medical Hospital from January 2019 to January 2022 were selected,and they were divided into the observation group(n=35)and the control group(n=35)according to the random number table method.The control group was treated with Iguratimod,while the observation group was treated with oral Chinese materia medica on the basis of the control group.The treatment continued for 12 weeks.The clinical efficacy,ESSDAI score,ESSPRI score,binocular Schirmer’s test,saliva flow rate and immunoglobulin G(IgG),interleukin-6(IL-6)and interleukin-17(IL-17)levels in the two groups of patients were compared.Results The total response rate of the observation group was superior to that of the control group,with statistically significant difference(P<0.05).After treatment,ESSDAI score,ESSPRI score,binocular Schirmer’s test and IgG,IL-6 and IL-17 levels of the two groups were all significantly improved compared with those before treatment,and the observation group was superior to those of the control group,with statistically significant differences(P<0.05).After treatment,the saliva flow rate of the observation group was significantly improved compared with that before treatment,with statistically significant difference(P<0.05),while that of the control group was not significantly improved compared with that before treatment(P>0.05).Conclusion Yangyin Qingfei Decoction,a Chinese materia medica,can significantly increase the clinical efficacy on patients with Sjogren’s syndrome,relieve the clinical symptoms and inflammatory indices of patients,promote the recovery of salivary gland and lacrimal gland functions,and regulate the abnormal immune response in vivo.
