摘　　要：江苏省常州市武进区自2016年以来,依托基本公共卫生服务项目,通过政府发动、扩大宣传,重点引导老年居民签约个性化服务包,并以此为抓手,强化基层能力建设,促进医防医养融合等,老年人个性化签约人数逐年增加,老年人健康管理质量不断提升。Since 2016,Wujin district, Changzhou, Jiangsu province relying on the basic public health services, through the government launched, expand propaganda, key guide the elderly residents signing personalized service package, and the gripper,strengthen the ability construction at the grassroots, promote the medical prevention medical fusion and so on, the elderly personalized signing number increased year by year, the elderly health management quality.
