摘 要:目的 观察慢性间歇性低氧(CIH)对阻塞性睡眠暂停低通气综合征(OSAHS)大鼠心肌细胞焦亡的影响。方法 将SD大鼠随机分为对照组、OSAHS组各8只。将OSAHS组置于间歇性低氧舱,建立OSAHS模型;对照组置于常氧舱内,提供常氧空气。8周后将大鼠处死,开胸取心脏。用扫描电子显微镜观察心肌细胞的微观结构,分别用实时荧光定量PCR法、Western blotting法检测心肌组织中焦亡关键蛋白(Caspase-1、GSDMD、NLRP3)mRNA、蛋白表达。结果 对照组心肌细胞膜完整、结构正常;OSAHS组心肌细胞膜完整性被破坏,表现为细胞膜表面欠光滑,形成裂隙、凹陷和大小不等的凸起,细胞膜表面有纤维组织附着,能够看到明显的泡状突起和焦亡小体流出。与对照组比较,OSAHS组Caspase-1、NLRP3 mRNA相对表达量高(P均<0.05),两组GDMDS mRNA相对表达量比较差异无统计学意义(P>0.05);与对照组比较,OSAHS组Caspase-1、GSDMD、GSDMD-N、NLRP3蛋白相对表达量高(P均<0.05)。结论 CIH可促进OSAHS大鼠心肌细胞焦亡。Objective To observe the effect of chronic intermittent hypoxia(CIH) on myocardial pyroptosis in rats with obstructive sleep apnea-hypopnea syndrome(OSAHS). Methods SD rats were randomly divided into the control group(8 rats) and OSAHS group(8 rats). Rats in the OSAHS group were placed in the intermittent hypoxia chamber and the OSAHS model was established. Rats in the control group were placed in the normoxic chamber and were provided with normoxic air. After 8 weeks, the rats were killed and the hearts were taken from the chest. Scanning electron microscope(SEM) was performed on the microstructure of myocardial cells. Real-time fluorescence quantitative PCR and Western blotting were used to detect the relative expression levels of apoptotic-related proteins(Caspase-1, GSDMD, and NLPR3) mRNAs and proteins in the myocardial tissues. Results In the control group, the myocardial cell membrane was intact and its structure was normal. The integrity of myocardial cell membrane in the OSAHS group was damaged, which showed that the surface of the cell membrane was not smooth, with cracks, pits and bulges of different sizes. There was fibrous tissues attached to the surface of the cell membrane, and obvious vesicular protrusions and scorched corpuscles could be seen flowing out. Compared with the control group, the relative expression levels of Caspase-1 and NLRP3 mRNA in the OSAHS group were higher(both P<0. 05);there was no significant difference in the GSDMD mRNA between OSAHS group and control group(P>0. 05). Compared with the control group, the relative protein expression levels of Caspase-1, GSDMD, GSDMD-N and NLRP3 in the OSAHS group were higher(all P<0. 05). Conclusion CIH can cause pyroptosis of cardiomyocytes in OSAHS rats.
