从石寿棠《医原》探讨燥湿的纲领性辨证及临床意义

摘　　要：清代名医石寿棠《医原》提出“燥湿二气为百病之纲领”,以燥湿二气统天地阴阳之气,将外感、内伤等诸多疾病归纳为燥湿二因,并以燥湿的不同表现解释病机。文章阐发石寿棠的燥湿理论及其临床实用意义,论述燥湿之病证的形成及其临床主要表现特征,探讨了燥湿之证的相互夹杂与相互转化的表现,并对燥湿真假证的辨识提出见解,同时阐述了燥湿之证与脾燥胃湿的密切关联,对燥湿病证的临床治疗大法作了要述。燥湿内容牵涉非常广泛,燥湿的临床辨证与八纲的寒热、虚实辨证有着密切的关系,从理论分析及实践表明,燥湿辨证列入八纲辨证,对临床更有纲领性的指导意义。SHI Shou-tang,a famous doctor in the Qing dynasty,put forward ’two qi of dryness and dampness are the outlines of all diseases’ in his book Yiyuan. The two qi of dryness and dampness unify the qi of heaven and earth,yin and yang. Many diseases such as external sensation and internal injury are classified as dryness and dampness,and the different manifestations of dryness and dampness explain the pathogenesis. This article expounds on the new theory of dryness and dampness and its clinical practical significance,this paper discusses the dryness of dampness disease formation and its main clinical features,discusses the dryness and dampness of the inclusions and the performance of the mutual transformation between each other,and put forward opinions to dryness and dampness false evidence identification,and expounds the dryness and dampness closely related with spleen and stomach wet,and the clinical treatment of dryness and dampness disease solution. The content of dryness and dampness is very extensive,and the clinical syndrome differentiation of dryness and dampness has a close relationship with the syndrome differentiation of cold and heat,deficiency and excess of eight categories. The theoretical analysis and practice indioated that dryness and dampness syndrome differentiation is list into eight priciple of syndrome differentiation,which has guiding significance.
