小柴胡汤方后注治咳加减法之六味柴胡汤的应用探讨

摘　　要：六味柴胡汤是《伤寒论》中小柴胡汤方后注的变方,张仲景用之治疗咳嗽。张立山教授认为方后注加减法亦是张仲景辨证论治原则的具体体现,将六味柴胡汤应用于临床,尤其是治疗呼吸系统疾病,疗效显著。六味柴胡汤的主要病机是少阳火郁的基础上兼微饮内停,方中柴胡、黄芩以和解少阳,半夏、干姜、五味子以温阳化饮止咳,炙甘草护胃和中。临床中可随证加减或合方应用,灵活变通。附验案一则以佐证。Liuwei Chaihu Decoction is a modified version of Xiaochaihu Decoction,which is from Shanghan Lun and ZHANG Zhong-jing uses it to treat coughs. Professor ZHANG Li-shan believes that the addition and subtraction is also a concrete embodiment of ZHANG Zhong-jing’s principle of dialectical treatment. Professor ZHANG Li-shan applied Liuwei Chaihu Decoction in clinical practice,especially in the treatment of respiratory system diseases,with remarkable effect. The main pathogenesis of Liuwei Chaihu Decoction is Shaoyang fire stagnation and mild fuid-retention stoppage. Liuwei Chaihu Decoction is to use Radix Bupleuri and Scutellariae Radix to reconcile Shaoyang,Rhizoma Pinelliae,Rhizoma Pinelliae and Fructus Schisandrae Chinensis to warm yang for resolving fluid retention to relieve cough,and prepared Radixet Glycyrrhizae to protect stomach and neutralize. It can be flexibly applied in clinic with syndrome addition or prescription combination. A test case is attached as evidence.
