产后情志异常的病名及其内涵辨析

摘　　要：产后出现精神障碍或情志异常多见于现代医学“产褥期抑郁症”或“产后抑郁”。目前此类病证存在病名使用不统一的情况,其中医内涵也尚未被深入挖掘。因此,笔者对现有病名进行比较研究,发现“产后情志异常”病名通俗易懂,可以统领涵盖中医历代古籍中有关产后神志和情志异常的多种病证,同时,“产后”“情志”二词也体现了中医的特色和内涵,从病名即能体现出病证特点,因而更加切合临床,故可作为统一使用的病名加以推广。通过历代医籍溯源,笔者将其中医内涵概括为“妇女分娩后气血亏虚,或复因瘀血、痰浊、外邪、郁热、七情过激、劳伤等引起以情志异常表现为特征,可伴随躯体症状的疾病”。Postpartum mental disorders or emotional abnormalities are more common in modern medicine of ‘puerperal depression’ or ‘postpartum depression’. At present,there is disunity in the use of disease names in this kind of disease syndrome,and its TCM connotation has not been excavated. Therefore,the authors conduct a comparative study of the existing disease names and found that the disease names of ‘postpartum emotional abnormality’ are easy to understand and can dominate a variety of diseases covering postpartum mental and emotional abnormalities in the ancient books of TCM in the past dynasty.At the same time,the words ‘postpartum’ and ‘emotional’ also reflect the characteristics and connotation of TCM,and can reflect the characteristics of the disease from the disease names,which is more in line with clinical practice,so it can be popularized as a standardized disease name. Through traceability of medical books in the past dynasties. The authors summarize its TCM connotation as ‘diseases characterized by emotional abnormalities due to deficiency of qi and blood after delivery in women,or due to blood stasis,phlegm,exogenous pathogens,stagnant heat,excessive seven emotions,and labor injury,which can be accompanied by somatic symptoms’.
