摘 要:目的:观察高压氧联合早期血管内栓塞治疗脑动脉瘤破裂出血患者的效果。方法:选取2019年4月至2021年4月该院收治的132例脑动脉瘤破裂出血患者进行前瞻性研究,采用随机数字表法将其分为对照组和观察组各66例。对照组接受血管内栓塞术治疗,观察组在对照组基础上联合高压氧治疗。治疗3个月后,比较两组临床疗效、治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数(BI)评分、血清炎性因子[降钙素原(PCT)、C反应蛋白(CRP)]水平、血管内皮功能指标[血管性血友病因子(vWF),内皮素-1(ET-1)、一氧化氮(NO)]水平,以及并发症发生率。结果:观察组治疗总有效率为87.88%(58/66),高于对照组的74.24%(49/66),差异有统计学意义(P<0.05);治疗后,观察组NIHSS评分低于对照组,BI评分高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组CRP、PCT、vWF和ET-1水平明显低于对照组,NO水平高于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为7.58%(5/66),低于对照组的19.70%(13/66),差异有统计学意义(P<0.05)。结论:高压氧联合早期血管内栓塞治疗脑动脉瘤破裂出血患者可提高治疗总有效率,减轻神经功能、血管内皮损伤程度,提高患者生活自理能力,并降低并发症发生率,效果优于单用血管内栓塞治疗。Objective:To investigate effects of hyperbaric oxygen combined with early endovascular embolization in treatment of patients with cerebral aneurysm rupture.Methods:A prospective study was conducted on 132 patients with cerebral aneurysm rupture hemorrhage diagnosed in this hospital from April 2019 to April 2021.They were divided into control group and observation group by using the random number table method,66 cases in each group.The control group was treated with endovascular embolization,while the observation group was treated with hyperbaric oxygen on the basis of that of the control group.After 3 months of treatment,the clinical efficacy,the National Institutes of Health stroke scale(NIHSS)score,Barthel index(BI)score,the serum inflammatory factor levels[procalcitonin(PCT),C-reactive protein(CRP)],and the vascular endothelial function index levels[von Willebrand factor(vWF),endothelin-1(ET-1),nitric oxide(NO)]were compared between the two groups before and after treatment,and the incidence of postoperative complications was recorded.Results:The total effective rate of the observation group was 87.88%(/66),which was higher than 74.24%(/66)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the NIHSS score of the observation group was lower than that of the control group,the BI score was higher than that of the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of CRP,PCT,vWF and ET-1 in the observation group were significantly lower than those in the control group,the level of NO was higher than that in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 7.58%(5/66),which was lower than 19.70%(13/66)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Hyperbaric oxygen combined with early endovascular embolization in the treatment of the patients with cerebral aneurysms can improve th
