阴道用乳杆菌活菌胶囊治疗Ⅰ、Ⅱ级宫颈上皮内瘤变患者的效果

摘　　要：目的:观察阴道用乳杆菌活菌胶囊治疗Ⅰ、Ⅱ级宫颈上皮内瘤变(CIN)患者的效果。方法:选取2019年1月至2021年3月该院收治的178例Ⅰ、Ⅱ级CIN患者进行前瞻性研究,以随机数字表法将其分为对照组87例和观察组91例。对照组采用干扰素α2a栓治疗,观察组在对照组基础上采用阴道用乳杆菌活菌胶囊治疗,比较两组临床疗效、人乳头瘤病毒(HPV)转阴率、阴道清洁度、CINⅠ逆转率、治疗前后T细胞亚群指标水平、随访半年内复发率和治疗期间不良反应发生率。结果:观察组治疗总有效率为94.51%(86/91),高于对照组的77.01%(67/87),差异有统计学意义(P<0.05);观察组HPV转阴率和阴道清洁度Ⅰ~Ⅱ级率均高于对照组,差异有统计意义(P<0.05);观察组CINⅠ逆转率为58.54%(48/82),高于对照组的43.04%(34/79),差异有统计学意义(P<0.05);治疗后,两组CD4^(+)和CD4^(+)/CD8^(+)水平均高于治疗前,且观察组高于对照组,两组CD8^(+)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组无复发,复发率低于对照组的16.42%(11/67),差异有统计学意义(P<0.05);两组治疗期间均未发生不良反应。结论:在干扰素α2a栓治疗基础上采用阴道用乳杆菌活菌胶囊治疗Ⅰ、Ⅱ级CIN患者可提高治疗总有效率、HPV转阴率、阴道清洁度和CINⅠ逆转率,改善T细胞亚群指标水平,降低复发率,效果优于单纯干扰素α2a栓治疗。Objective:To observe effects of live Lactobacillus capsule for vaginal use in treatment of patients with cervical intraepithelial neoplasia(CIN)grade I and II.Methods:A prospective study was conducted on 178 patients with CIN I and II admitted to the hospital from January 2019 to March 2021.They were divided into control group(87 cases)and observation group(91 cases)by using the random number table method.The control group was treated with interferonα2a suppository,while the observation group was treated with live Lactobacillus capsule for vaginal use on the basis of that of the control group.The clinical efficacy,the human papillomavirus(HPV)negative conversion rate,the vaginal cleanliness,the CIN grade I reversal rate,the T cell subset levels before and after treatment,the recurrence rate within half a year of follow-up,and the incidence of adverse reactions during the treatment were compared between the two groups.Results:The total effective rate of treatment in the observation group was 94.51%(86/91),which was higher than 77.01%(67/87)in the control group,and the difference was statistically significant(P<0.05).The HPV negative conversion rate and the vaginal cleanliness grade I-II rate in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The CIN grade I reversal rate in the observation group was 58.54%(48/82),which was higher than 43.04%(34/79)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before the treatment,and those in the observation group were higher than those in the control group;the levels of CD8^(+)in the two groups were lower than those before the treatment,and that in the observation group was lower than that in the control group;and the differences were statistically significant(P<0.05).There was no recurrence in the observation group,the recurrence rate was lower than 16.42%(11/67)in the co
