金朝晖基于“络以通为用”理论论治慢性阻塞性肺疾病稳定期经验

摘　　要：金朝晖教授擅长诊治各类肺系疾病,通过长期临床实践,基于叶天士“络以通为用”理论,认为慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)稳定期,肺虚络瘀为发病之本,邪积肺络为病机关键,虚实夹杂为病理特点,确立通络法为主要治则。通过辨证与辨病相结合,提出补虚通络法、祛邪通络法。临证时视患者具体情况,配合补肺、健脾、益肾、清热、养阴、散寒、祛痰、活血等治法,在临床诊疗中收获良好反响。并附验案1则佐证。JIN Zhaohui is good at diagnosing and treating various pulmonary diseases.Through long-term clinical practice,based on YE Tianshi’s theory of“connecting collaterals for use”,he believes that chronic obstructive pulmonary disease(COPD)is in a stable period.Lung deficiency and blood stasis are the root of the disease,evil accumulation of lung collaterals is the key to the pathogenesis,and deficiency and excess are the pathological characteristics,and the method of dredging collaterals is established as the main treatment principle.Through the combination of syndrome differentiation and disease differentiation,the methods of tonifying deficiency and dredging collaterals and eliminating evils and dredging collaterals are proposed.Depending on the specific condition of the patient in clinical practice,it is combined with treatments such as nourishing the lung,invigorating the spleen,nourishing the kidney,clearing heat,nourishing yin,dispelling cold,eliminating phlegm,and promoting blood circulation,and has received good responses in clinical diagnosis and treatment.Attached is test case 1 supporting evidence.
