摘　　要：目的探讨临床药师参与骨科围手术期下肢深静脉血栓形成抗凝治疗并实施药学服务的方法。方法外科临床药师团队根据国内外指南和临床治疗经验,建立骨科围手术期下肢深静脉血栓形成抗凝治疗路径,并介绍3例基于上述治疗路径开展临床药学服务的典型案例。结果基于指南并结合疾病特点建立的药物治疗路径,有利于规范骨科围手术期下肢深静脉血栓形成的抗凝治疗,提升医疗质量安全。结论该路径是外科临床药师对骨科围手术期下肢深静脉血栓形成患者开展有针对性药学服务的有效模式,可供外科药师同行参考。Objective To explore ways in which clinical pharmacists participate in perioperative anticoagulant treatment of lower extremity deep venous thrombosis(DVT)in orthopedics patients and deliver pharmaceutical care.Methods According to guidelines at home and abroad,a clinical pharmacist team for anticoagulation established a perioperative anticoagulant therapy pathway for lower extremity DVT in orthopedics patients before they presented three typical cases where they had given clinical pharmaceutical care based on the above pathway.Results The pathway established based on the guidelines and symptoms of the disease helped standardize perioperative anticoagulant treatment of lower extremity DVT in orthopedics patients and improved the quality and safety of health care.Conclusion This pathway is an effective model by which surgical clinical pharmacists can carry out targeted pharmaceutical care for lower extremity DVT patients during the orthopedic perioperative period,which should be made more accessible in hospitals.
