摘　　要：目的分析家庭医生签约服务的社区老年人健康状况以及老年人健康管理效果影响因素,以期为提升老年人健康管理水平提供建议。方法选取某社区2019—2020年60岁以上参加健康体检并与家庭医生签约的老年人为研究对象,通过问卷调查与健康宣教结合的方式展开。干预组根据项目要求实施为其12个月健康宣教以及指标监测。对照组接受常规管理服务。结果干预12个月后,两组在健康行为评分、第一秒用力呼气容积/用力肺活量的上升和舒张压、尿微量白蛋白和尿肌酐比值、糖化血红蛋白的比较上,差异有统计学意义(均P<0.05)。年龄、依从性、BMI对老年人健康管理效果的影响因素差异有统计学意义(均P<0.05),其中年龄和BMI起到负向作用。结论家庭医生签约服务的老年人健康管理服务对维护老年人的健康有促进作用,应根据不同人群特点开展有针对性的服务。Objective To analyze the health status of the elderly in community contracted by family doctors and the influencing factors of the health management effect of the elderly,so as to provide suggestions for improving the health management level of the elderly.Methods The elderly over 60 years old in a community from 2019 to 2020 who participated in physical examination and signed a contract with a family doctor were selected as the research objects,and the research was carried out through questionnaire survey and health education.The intervention group implemented health education and indicator monitoring for 12 months according to the project requirements.The control group received routine management services.Results After12 months of intervention,there were significant differences between the two groups in health behavior scores,the rise of forced expiratory volume/forced vital capacity in the first second,diastolic pressure,the ratio of urinary microalbumin to creatinine,and glycosylated hemoglobin(all P<0.05).The influencing factors of age,compliance and BMI index on the health management effect of the elderly were statistically significant(all P<0.05).Among them,age and BMI index play a negative role.Conclusion The elderly health management service signed by family doctors has a positive role in promoting the health of the elderly,and targeted services should be carried out according to the characteristics of different populations.
