摘  要:目的观察推拿点按法对胶质细胞源性神经营养因子在中风后肢体痉挛大鼠脑组织中表达的影响。方法将40只SD大鼠随机分为假手术10只、模型和推拿组各15只,采用线栓法建立大鼠中动脉闭塞模型,造模成功后随机分为模型组和推拿组。推拿组点按大鼠头穴顶区、两侧夹脊穴及患侧曲池、阳陵泉各1次,治疗7 d,其余2组不予处理。记录治疗前后Zealonga及改良Ashworth评分,网屏实验评分等行为学数据。Tunel荧光染色观察大鼠脑皮质神经细胞凋亡程度,蛋白免疫印迹法及聚合酶链式反应检测大鼠脑皮质中GDNF蛋白表达。结果行为学检测结果显示,与假手术组比较,模型组大鼠改良Ashworth评分增高,网屏实验评分升高;与模型组比较,推拿组大鼠改良Ashworth评分降低,网屏实验评分降低。模型组大鼠脑皮质神经细胞凋亡指数明显高于推拿组及假手术组。推拿组GDNF蛋白表达含量较模型组增加;推拿组的GDNF mRNA表达高于模型组。结论推拿手法可通过促进GDNF的表达来调节神经细胞凋亡情况,减少脑皮质神经元损伤,降低中风后肢体痉挛大鼠的痉挛程度。Objective To observe the effect of massage and finger pointing manipulations on the expression of glial cell-derived neurotrophic factors in the brain tissues of rats with limb spasm after stroke.Methods Forty Sprague Dawley(SD)rats were randomly divided into a sham operation group(10 rats),a model group(15 rats)and a massage group(15 rats).A middle artery occlusion model was established by the thread occlusion method.After successful modeling,they were randomly divided into a model group and a massage group.The rats in the massage group were treated for 7 days by pressing the top area of the cephalic point,both sides of Jiaji point and Baihui,Quchi,and Yanglingquan points once daily,while the other two groups were not treated.The behavioral data such as Zealonga and modified Ashworth scores,and screen test scores were recorded before and after treatment.Tunel fluorescence staining was used to observe the degree of neuronal apoptosis.Western blot and polymerase chain reaction were used to detect the protein expression of GDNF in rat cerebral cortex.Results The behavioral test results showed that compared with the sham operation group,the modified Ashworth score and the screen test score of the model group were increased,while compared with the model group,the modified Ashworth score and the screen test score of the rats in the massage group were decreased.The apoptosis index of cortical neurons in the model group was significantly higher than those in the massage group and the sham operation group.The protein expression of GDNF in the massage group was higher than that in the model group.The mRNA expression of GDNF in the massage group was higher than that in the model group.Conclusion Massage can reduce the degree of spasticity in rats with post-stroke limb spasm,which may regulate the apoptosis of nerve cells by promoting the expression of GDNF,reduce the damage of neurons in the cerebral cortex,and improve the neurological injury in rats.
