2型糖尿病合并阻塞性睡眠呼吸暂停综合征的临床特征分析

摘　　要：目的探索2型糖尿病合并阻塞性睡眠呼吸暂停综合征(OSAS)患者的临床特征。方法采集100例2型糖尿病合并阻塞性睡眠呼吸暂停综合征患者信息,逐一辨证分型及中药治疗,分析证型比例以及各证型之间和治疗前后体质量指数(BMI)、空腹血糖(FPG)、餐后血糖(PPG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)、呼吸暂停低通气指数(AHI)、Epworth嗜睡量表评分(ESS)的差异。结果热盛伤津证最多,其次是气阴两虚证,阴阳两虚证最少。2型糖尿病合并阻塞性睡眠呼吸暂停综合征患者的AHI与BMI成正相关。中药治疗后患者的FPG、PPG、HbA1c、HOMA-IR及ESS评分下降(P<0.05)。结论热盛津伤证是2型糖尿病合并阻塞性睡眠呼吸暂停综合征患者最多见的证型,其次是气阴两虚证、痰热互结证、肝肾阴虚证,阴阳两虚证则最少,中药治疗可使2型糖尿病合并阻塞性睡眠呼吸暂停综合征患者临床指标改善。Objective To explore the clinical characteristics of type 2 diabetes patients combined with the obstructive sleep apnea syndrome(OSAS).Methods The information of 100 patients with type 2 diabetes complicated with the obstructive sleep apnea syndrome was collected,and their syndrome differentiation and traditional Chinese medicine treatment were performed one by one.The proportion of their syndrome types and the differences in their body mass index(BMI),fasting plasma glucose(FPG),postprandial plasma glucose(PPG),glycosylated hemoglobin(HbA1c),insulin resistance index(HOMA-IR),apnea hypopnea index(AHI)and the scores of Epworth Sleeping Scale(ESS)were analyzed between different syndrome types and before and after treatment.Results In patients of type 2 diabetes mellitus with OSAS,the proportion of those with the syndrome of consumption offl uid due to intense heat excess heat was the highest,followed by the syndrome of defi ciency of both qi and yin,while the syndrome of defi ciency of both yin and yang was the least.AHI was positively correlated with BMI in patients with type 2 diabetes mellitus and OSAS.The scores of FPG,PPG,HbA1c,HOMA-IR and ESS were decreased after a traditional Chinese medicine treatment(P<0.05).Conclusion In the patients with type 2 diabetes combined with OSAS,the syndrome of consumption offl uid due to intense heat excess heat is the most common type,followed by the syndrome of defi ciency of both qi and yin,the syndrome of intermingled phlegm and heat,the syndrome of yin defi ciency of liver and kidney,while the syndrome of defi ciency of both yin and yang is the least.The traditional Chinese medicine treat can improve the clinical indicators of patients with type 2 diabetes mellitus combined with OSAS.
