摘　　要：IgA肾病是临床难治性免疫相关性肾小球疾病,发病机制尚未明确,因此缺乏特异性治疗方法。中医辨证论治的优势在于通过改善IgA肾病患者体质状态,控制诱发加重因素,阻断病程迁延发展,保护肾脏功能,从而改善患者预后,具有积极的临床意义。本文基于玄府理论,认为肺脾肾“玄府郁闭”是IgA肾病的核心病机,立“开玄通府”治疗原则,以补虚固玄、祛风开玄、除湿开玄、化瘀开玄为法,以期为临床治疗IgA肾病提供新思路。IgA nephropathy is a clinically refractory immune-related glomerular disease,and the pathogenesis is not yet clear,so there is a lack of specific treatment.The advantage of TCM syndrome differentiation and treatment lies in improving the patients’prognosis by improving the physical state of IgA nephropathy patients,controlling the aggravating factors,blocking the protracted development of the disease course,and protecting the kidney function,with positive clinical significance.Based on the Xuanfu theory,this article believed that“closure of Xuanfu”of the lung,spleen and kidney is the core pathogenesis of IgA nephropathy,and established the treatment principle of“opening Xuanfu”,which aimed to“tonifying deficiency and strengthening Xuanfu”,“dispelling wind and opening Xuanfu”,“dissipating dampness and opening Xuanfu”and“removing blood stasis and opening Xuanfu”,in order to provide new ideas for clinical treatment of IgA nephropathy.
