摘　　要：目的研究带状疱疹后遗神经痛血脉瘀阻证应用中医外治综合治疗的临床效果。方法将我院2021年1月-12月收治的60例带状疱疹后遗神经痛患者随机分为对照组和观察组各30例,对照组给予普瑞巴林口服,观察组采用超声定位脊神经出口下针刀松解术辅以针刺及中药敷贴。对比2组疗效、疼痛视觉模拟评分法(VAS)评分。对比2组治疗前后的阿森斯失眠量表评分(AIS)、皮肤病生活质量量表评分(DQOLS)及C反应蛋白(CRP)、白细胞介素-8(IL-8)、肿瘤坏死因子α(TNF-α)水平。观察2组不良反应。结果观察组总有效率显著高于对照组(P<0.05)。2组治疗前疼痛VAS评分差异无统计学意义(P>0.05);治疗后观察组VAS评分低于对照组,差异有统计学意义(P<0.05)。2组治疗前AIS、DQOLS、CRP、IL-8、TNF-α对比,差异无统计学意义(P>0.05);治疗后观察组均优于对照组,差异有统计学意义(P<0.05)。2组均无显著不良反应。结论带状疱疹后遗神经痛血脉瘀阻证应用中医外治综合治疗效果显著,可显著减轻疼痛,提升生活质量,遏制炎症反应,安全性高。Objective To study the clinical effect of comprehensive treatment of TCM external therapy for postherpetic neuralgia syndrome of blood stasis.Methods 60 patients with postherpetic neuralgia diagnosed and treated in our hospital from January 2021 to December 2021 were randomly divided into the control group and the observation group,30 cases in each group. The control group was given pregabalin orally,and the observation group was given acupuncture and acupuncture and traditional Chinese medicine applicator under ultrasound positioning of the spinal nerve outlet. The curative effect and VAS of pain were compared between the two groups. The AIS,DQOLS,CRP,IL-8,TNF-αlevels. The adverse reactions of the two groups were observed.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05). There was no significant difference in VAS scores of pain between the two groups before treatment(P>0.05);After treatment,the VAS score of the observation group was lower than that of the control group,with a statistically significant difference(P<0.05). There was no significant difference in AIS,DQOLS,CRP,IL-8 and TNF-a between the two groups before treatment(P>0.05);After treatment,the observation group was superior to the control group,and the difference was statistically significant(P<0.05). There was no significant adverse reaction in both groups.Conclusion Postherpetic neuralgia with blood stasis syndrome has a significant effect on the comprehensive treatment of TCM external therapy,which can significantly reduce pain,improve the quality of life,and curb the inflammatory response,with good safety.
