正常母胎血浆钙、镁、钾和钠含量的关系及影响因素

摘  要:目的研究常量金属元素钙、镁、钾和钠在正常孕妇静脉血及胎儿脐血中的分布规律,评价各元素胎盘传输效能(PTE,某元素脐血浓度/母血浓度)及影响因素。方法随机选取200对来源于兰州地区出生队列样本库的生物样本入组,检测分娩前母血和新生儿脐血中钙、镁、钾和钠元素浓度并进行相关性分析,分析各元素的PTE及其影响因素。结果母全血与脐血血钙、镁、钠、钾元素中值浓度分别为9.93和9.59mg/L、29.44和30.35mg/L、441.92和429.51 mg/L、686.02和896.35 mg/L;钙元素和镁元素、钾元素和镁元素显著正相关;血钾元素PTE高(PTE>1),而血钙、镁、钠元素母胎界面PTE无明显差异;胎膜早破对钾元素PTE会产生影响(P<0.05),分娩方式、产时使用缩宫素和生理盐水对钠元素的PET也有影响(P<0.05);而其余因素对血钙、镁、钠、钾元素PTE的影响均差异无统计学意义(P>0.05)。结论产时个体间血钙、镁、钾和钠元素PTE离散度大,钾元素PTE高,谨防输注过量问题;胎膜早破、分娩方式及产时药物输注对PTE会造成影响。Objective To study the distribution of human macrometal elements Ca,Mg,K and Na in the peripheral blood of pregnant women and cord blood of newborns,and to evaluate the placental transfer efficiency(PTE,concentration of umbilical blood of a certain element/concentration of maternal blood)and influencing factors of each element.Methods 200pairs of biological samples were randomly selected from the birth cohort biological sample bank of Lanzhou.The concentrations of Ca,Mg,K and Na in maternal peripheral whole blood and neonatal cord blood were detected,the correlation of elements between maternal and cord blood were analyzed,and the PTE of each element and its influencing factor were analyzed.Results The median concentrations of Ca,K,Mg,and Na in maternal whole blood and cord blood were 9.93 and 9.59 mg/L,29.44 and 30.35 mg/L,441.92 and 429.51 mg/L,686.02 and 896.35 mg/L,respectively.There were significant positive correlations between Ca and Mg,K and Mg.There was no significant difference in PTE of blood Ca,Mg and Na,but the PTE of blood K was high(PTE>1)(P<0.05).In addition to the statistically significant difference in the effect of premature rupture of membranes on blood K PTE(P<0.05),different mode of delivery,the use of oxytocin and normal saline during delivery on blood Na PET(P<0.05).There was no significant difference in the influence of other factors on the PTE of blood Ca,Mg,K,Na(P>0.05).Conclusion The PTE dispersion of Ca,Mg,K and Na in the blood among individuals were large.The blood K PTE is high,so beware of the problem of over-infusion.Premature rupture of membranes,different mode of delivery and intravenous infusion of drugs have significant effect on PTE.
