小儿隐睾扭转7例诊治分析并文献复习

摘　　要：目的 探讨小儿隐睾扭转的临床特点及诊治要领。方法 回顾性分析2015年1月至2020年10月遵义医科大学附属医院收治的7例小儿隐睾扭转的临床资料,同时进行相关文献复习。结果 病儿发病年龄中位数为9岁,范围为2个月至13岁;发病至手术时间的中位数为54 h,范围为14~102 h;首诊误诊3例;5例表现为一侧腹股沟区肿痛,其余2例分别以拒乳、哭闹和左下腹痛为主,查体均触及一侧腹股沟区包块及同侧阴囊空虚。所有病例均行彩色多普勒超声(CDFI)检查和手术探查,两者结果均相符。术中证实均为左侧腹股沟隐睾扭转,其中手术切除者5例,保留者2例。结论 应增强家长及医生对隐睾扭转的认识和重视度。对于阴囊空虚伴有腹股沟区包块疼痛或下腹疼痛者需警惕此病,并及时就诊,全面查体,常规予CDFI检查,对高度怀疑者,尽早手术探查。Objective To explore the clinical characteristics, diagnosis and treatment of torsion of cryptorchidism in children.Methods The clinical data of 7 cases with torsion of cryptorchidism in children admitted to the Affiliated Hospital of Zunyi Medical University from January 2015 to October 2020 were retrospectively analyzed, and the relevant literature was reviewed.Results The median age of onset was 9 years(2 months to 13 years). The median time from onset to operation was 54 h(14-102 h). 3 cases were misdiagnosed in the first diagnosis. 5 cases presented swelling and pain in one side of the groin area, while the remaining 2 cases presented mainly breast rejection, crying and left lower abdominal pain, respectively. Physical examination revealed lumps in one side of the groin area and emptiness in the ipsilateral scrotum. Color Doppler ultrasonography(CDFI) and surgical exploration were performed in all cases, and the results were consistent. Intraoperative torsion of left inguinal cryptorchidism was confirmed in all cases, of which 5cases were excised and 2 cases were retained.Conclusions Parents and doctors should enhance their awareness and attention to torsion of cryptorchidism. For patients with scrotal emptiness accompanied by groin mass pain or lower abdominal pain, it is necessary to be alert to this disease, timely medical treatment, comprehensive physical examination, routine CDFI examination, for highly skeptical patients, early surgical exploration.
