冠心病致心力衰竭患者游离甲状腺激素水平的变化

摘　　要：目的探讨冠心病致心力衰竭(心衰)患者游离甲状腺激素水平的变化。方法回顾性分析59例冠心病致心衰患者(观察组)和同期住院的88例冠心病非心衰患者(对照组)临床资料。检测并比较两组患者血脂、N末端B型脑钠肽前体(NT-proBNP)以及游离甲状腺激素TSH、FT_(3)、FT_(4)水平。采用彩色多普勒超声检测两组患者左心室射血分数(LVEF)。结果观察组LVEF低于对照组,而NT-proBNP水平高于对照组(P<0.05)。观察组FT_(3)水平低于对照组,而FT_(4)水平高于对照组(P<0.05);两组TSH水平比较差异无统计学意义(P>0.05)。Spearman相关性分析结果显示,FT_(3)与LVEF呈正相关(r=0.391,P<0.01),与NT-proBNP呈负相关(r=-0.433,P<0.01);FT_(4)与LVEF呈负相关(r=-0.182,P<0.05),与NT-proBNP呈正相关(r=0.325,P<0.01)。结论冠心病患者低FT_(3)和高FT_(4)有存在心衰可能;联合监测游离甲状腺激素与NT-proBNP水平,有助于冠心病致心衰患者的诊治。Objective To investigate the changes of free thyroid hormone levels in the patients with heart failure(HF)caused by coronary heart disease(CHD).Methods The clinical data of 59 patients with HF caused by CHD(group A)and 88 CHD patients without HF(group B)hospitalized at the same time were analyzed retrospectively.The levels of blood lipid,N-terminal pro-B-type natriuretic peptide(NT-proBNP)and free thyroid hormones TSH,FT_(3)and FT_(4)were measured and compared between the two groups.Left ventricular ejection fraction(LVEF)of the two groups was measured by color Doppler echocardiography.Results LVEF in group A was lower than that in group B,while NT-proBNP in group A was higher than that in group B(P<0.05).FT_(3)in group A was lower than that in group B,while FT_(4)was higher than that in group B(P<0.05).There was no significant difference in TSH between the two groups(P>0.05).Spearman correlation analysis showed that FT_(3)was positively correlated with LVEF(r=0.391,P<0.01)and negatively correlated with NT-proBNP(r=-0.433,P<0.01).FT_(4)was negatively correlated with LVEF(r=-0.182,P<0.05)and positively correlated with NT-proBNP(r=0.325,P<0.01).Conclusion The CHD patients with low FT_(3)and high FT_(4)may have HF.Combined monitoring of free thyroid hormone levels and NT-proBNP is helpful for the diagnosis and treatment of the patients with HF caused by CHD.
