摘  要:目的探讨脉冲振荡技术(IOS)联合呼出气一氧化氮(FeNO)测定在3~6岁咳嗽变异性哮喘(CVA)患儿的应用价值。方法根据诊断标准将240例3~6岁慢性咳嗽患儿分为CVA组和非CVA组,每组120例。比较两组FeNO以及IOS相关指标,包括振荡频率在5 Hz时的呼吸总阻抗(Z5)、共振频率(Fres)、振荡频率在5 Hz时的气道粘性阻力(R5)、振荡频率在20 Hz时的气道阻力(R20)和振荡频率在5Hz时的电抗(X5)。结果CVA组FeNO、Z5、Fres、R5、R20和X5均高于非CVA组(P<0.05)。CVA组FeNO与Fres呈正相关(r=0.133,P=0.042)。各指标联合检测诊断的特异度和曲线下面积均高于单独检测(P<0.05)。结论IOS联合FeNO测定对3~6岁CVA患儿有一定的诊断价值。Objective To investigate the application value of impulse oscillometry system(IOS)combined with fractional exhaled nitric oxide(FeNO)in the 3-6 years old children with cough variant asthma(CVA).Methods According to the diagnostic criteria,a total of 240 children with chronic cough were randomly divided into groups of CVA and non-CVA with 120 cases each.FeNO value and IOS-related indexesincluding airway impedance at 5 Hz(Z5),resonance frequency(Fres),airway resistance at 5 Hz(R5),airway resistance at 20 Hz(R20)and reactance at 5 Hz(X5)were compared between the two groups.Results FeNO,Z5,Fres,R5,R20 and X5 in group CVA were higher than those in group non-CVA(P<0.05).FeNO was positively correlated with Fres in group CVA(r=0.133,P=0.042).The specificity and AUC of combined detection of all indexes were higher than those of single detection(P<0.05).Conclusion IOS combined with FeNO has certain diagnostic value in the children with CVA aged 3-6 years old.
