临床护士多点执业的意愿和动机调查

摘　　要：目的了解临床护士多点执业的意愿和动机,为护士多点执业的推广提供参考。方法采取便利抽样法,利用网络电子问卷对甘肃省630名临床护士多点执业的意愿、动机进行调查。结果有405人(64.29%)对多点执业持认同态度,年龄>40~50岁、护龄>10年、在二级医院工作、月工资5000~8000元、内科及外科护士较其他特征的护士对多点执业认同度更高(均P<0.05)。护士多点执业的主要动力因素为提高个人经济收入(79.52%)和为患者提供便捷的护理服务(75.24%),主要阻力因素为缺乏政策支持(64.29%)和无职业责任保险(63.02%)。认为多点执业的最大益处是能够增加护士合理收入(81.27%),最大弊端是助长职工趋利行为(75.71%);选择人数最多的多点执业形式为兼职在基层开设专科护理门诊(79.05%)。结论临床护士对多点执业认同度较高,对其利弊有清楚的认知,但执业保障体系的缺失对护士多点执业意愿产生了不利影响。建议相关部门建立健全法律法规、制度和行业标准,为护士多点执业提供有力保障。Objective To understand the willingness and motivation of clinical nurses’multi-sited license and to provide reference for the promotion of multi-sited license.Methods Convenient sampling method was adopted to investigate the willingness and motivation of 630 clinical nurses’multi-sited license in Gansu province by using online electronic questionnaire.Results There were405(64.29%)people who agreed with multi-sited license,aged between 40 to 50 years,nursing age more than 10 years,working in second-class hospital and monthly salary between 5000 to 8000 yuan.After analyzing,internal medical and surgical nurses had higher recognition of multi-sited license than nurses with other characteristics(average of P<0.05).The main driving factors for nurses’multi-sited license were improving personal economic income(79.52%)and providing convenient nursing services for patients(75.24%),while the main resistance factors were lack of policy support(64.29%)and no professional liability insurance(63.02%).It is believed that the biggest benefit of multi-sited license is to increase the reasonable income of nurses(81.27%),and the biggest disadvantage is the tendency of profit-seeking behavior(75.71%);The multi-sited license with the largest number of people was part-time specialized nursing clinics at the primary hospital(79.05%).Conclusion Clinical nurses have identity for multi-sited license and a clear understanding of its advantages and disadvantages,but the lack of practice guarantee system has a negative impact on nurses’willingness of multi-sited license.It is suggested that relevant departments should establish the sound laws,regulations,systems and industry standards to provide favorable guarantee for nurses’multi-sited license.
