加味当归四逆汤联合针刺外洗综合疗法治疗糖尿病周围神经病变(阳虚寒凝证)的临床评价

摘　　要：目的 分析加味当归四逆汤联合针刺、中药外洗综合疗法对糖尿病周围神经病变阳虚寒凝证患者的临床疗效。方法 将80例糖尿病周围神经病变阳虚寒凝证患者,用数字随机表法随机分为治疗组和对照组,每组各40例。两组在控制血糖等基础治疗上,对照组服用甲钴胺,治疗组采用加味当归四逆汤联合针刺、外洗综合治疗,疗程8周。观察两组患者临床治疗效果,并对两组治疗前后中医证候积分、多伦多临床评分(TCSS)、神经传导速度及血液流变学指标进行比较。结果 治疗组的总有效率为92.50%,对照组的总有效率为72.50%,治疗组总有效率明显高于对照组(P<0.05);治疗后,治疗组患者的中医证候积分和TCSS积分呈明显降低趋势;治疗组的积分明显低于对照组(P<0.05);治疗组患者神经传导速度、血液流变学指标改善均明显优于对照组(P<0.05)。结论 加味当归四逆汤联合针刺、中药外洗综合治疗糖尿病周围神经病变的疗效确切,能有效改善患者的临床症状、提高神经传导速度,改善血液流变学指标。Objective To analyze the clinical effect of Modified Danggui Sini decoction combined with acupuncture and external washing of traditional Chinese medicine on diabetic peripheral neuropathy.Methods 80 cases of diabetic peripheral neuropathy with Yang deficiency and cold coagulation syndrome were randomly divided into treatment group and control group,40 cases in each group.On the basis of controlling blood glucose,the control group was treated with Mecobalamin,and the treatment group was treated with modified Angelica Sini decoction combined with acupuncture and external washing for 8 weeks.To observe the clinical treatment effect of the two groups,and to compare the TCM syndrome score,Toronto clinical score(TCSS),nerve conduction velocity and hemorheology indexes before and after treatment.Results The total effective rate of the treatment group was 92.50%,the total effective rate of the control group was 72.50%,the total effective rate of the treatment group was significantly higher than that of the control group(P< 0.05);after treatment,the TCM syndrome integral and TCSs integral of the treatment group were significantly reduced;the integral of the treatment group was significantly lower than that of the control group(P< 0.05);the improvement of nerve conduction speed and hemorheology index of the treatment group was significantly better than that of the control group Group A(P< 0.05).Conclusion The therapeutic effect of Modified Danggui Sini decoction combined with acupuncture and external washing of traditional Chinese medicine on diabetic peripheral neuropathy is definite.It can effectively improve the clinical symptoms,nerve conduction velocity and hemorheology indexes of patients.
