摘  要:退行性腰椎管狭窄症以神经受压为特征,发病与年龄增长、腰椎老化退变、椎体后缘与关节突增生有关,是导致中老年人腰腿痛的常见病因。经皮椎间孔镜手术是在内镜技术辅助下,对脊柱病变部位操作实现最小的损伤,是治疗本病的主要微创技术,创伤小、恢复快、疗效可靠。本文对近年经皮椎间孔镜下腰椎管减压的技术演进展开综述,以时间为轴,系统阐述包括杨氏内镜脊柱系统、经皮椎间孔脊柱内镜技术、改良的杨氏技术、靶向穿刺技术、经椎板间隙入路技术、倍斯技术、经椎板下内镜外科手术技术、德尔塔脊柱微创技术、双通道套叠技术在内的经皮椎间孔镜技术方法与临床应用情况,并阐述手术适应证,分析未来前景。Degenerative lumbar spinal stenosis is characterized by nerve compression,its onset is related to age,aging and degeneration of lumbar spine,and proliferation of posterior edge of vertebral body and articular process,it is a common cause of low back and leg pain in middle-aged and old people.Percutaneous foraminal endoscopic surgery is a major minimally invasive technique for the treatment of this disease,which can achieve minimal damage to the spinal lesion with the assistance of endoscopic technology,it has small trauma,fast recovery and reliable efficacy.This article reviewed the technical evolution of percutaneous intervertebral foraminal endoscopic for lumbar spinal canal decompression in recent years,taking time as the axis,it systematically expounded the Yeung endoscopic spine system,percutaneous intervertebral foraminal spinal endoscopic technology,improved Yeung technology,targeted puncture technology,trans laminar space approach technology,times technology,trans laminar endoscopic surgery technology,delta spinal minimally invasive technology methods and clinical application of percutaneous intervertebral foraminal endoscopic including double channel intussusception technology are introduced,and the surgical indications are described,and the future prospects are analyzed.
