中兽医治疗猫慢性肾衰病例报告

摘　　要：慢性肾功能衰竭是临床中老年猫常见的一种进展性、不可逆、高致死率的严重疾病。西兽医对猫慢性肾衰采用对症治疗,患猫肌酐值越高,该病的临床分期等级越高,预后越差。在中兽医辨证中,肾阳虚为猫慢性肾功能衰竭主要病因病机,本虚标实,虚实夹杂,在病程不同阶段可表现为不同证型,且可累及脾、肺等脏腑。本文报告了一例猫慢性肾衰病例,中兽医辨证为脾肾阳虚兼脾肾气虚,伴血瘀、水湿之证,组方原则为温阳利水,健脾补肾,益气养阴,活血和络,并随证调方。通过8次治疗,患猫尿素氮水平由174.0 mg/dL降至90.0 mg/dL,血清肌酐水平由4.0 mg/dL降至2.1 mg/dL,患猫精神好转,食欲改善,排便、排尿正常,取得了良好疗效,不但延缓了肾损伤进程,而且明显提高了患猫生活质量,为临床中兽医诊疗该病提供了参考。Chronic renal failure(CRF) is a severe, progressive and irreversible disease with high mortality, which mostly happened in elderly cats. The main treatment principle of western veterinarians for CRF in cats is symptomatic treatment. The higher the creatinine level of the sick cat, the higher the clinical stage and the worse the prognosis of the disease. According to traditional Chinese veterinary medicine(TCVM) syndrome differentiation, CRF in cats is mainly caused by kidney Yang deficiency, and it is a disease with deficiency in the root and excess in the tip, and also it is a mixture of deficiency and excess. In different stages of the course, the disease can be manifested as different syndromes. CRF may also involve damage in spleen, lung and other Zangfu organs. Under the guidance of the Holism, the key point of TCVM treatment is to regulate and restore the balance of Zangfu organs, Qi and blood, Yin and Yang. According to the specific symptoms and syndrome type, different self-designed Chinese medicine prescription is adjusted. This paper reported a case of feline CRF diagnosed and treated by TCVM. The cat was diagnosed as spleen and kidney Qi and Yang deficiency, accompanied by blood stasis and water dampness by TCVM syndrome differentiation. The traditional Chinese medicine prescription was formulated according to the principle of warming Yang to promote diuresis, invigorating spleen and kidney, nourishing Qi and Yin, and promoting blood circulation, and the prescription was adjusted to changes of the cat’s clinical symptoms. After 8 times of TCVM treatment, the cat’s urea nitrogen decreased from 174.0 mg·dL-1to 90.0 mg·dL-1, creatinine decreased from 4.0 mg·dL-1to 2.1 mg· dL-1, the cat’s spirit and appetite improved, and its urine and feces were back to normal, which meant the curative effect of TCVM was satisfying, it not only delayed the process of kidney injury, but also significantly improved the quality of life of the cat, which could provide a reference for clinical diagnosis and treatment
