摘 要:目的 分析2019年长三角区域(上海市、江苏省、浙江省、安徽省)HIV/AIDS患者的流动情况及其影响因素,为该区域制定有针对性的防控措施提供依据。方法 在艾滋病综合防治信息系统中,收集2019年长三角地区新报告的HIV/AIDS患者的基本人口学特征及户籍地、报告地和现住址情况。采用多因素Logistic回归分析HIV/AIDS患者流动的影响因素。结果 2019年长三角地区报告的14 048例HIV/AIDS患者中,流动病例为4 831例,占34.39%。上海市报告的流动病例占比最高(46.58%),安徽省报告的流动病例占比最低(12.99%)。流动的病例中以省际流动的为主,上海市、江苏省、浙江省、安徽省的比例分别为41.18%、20.33%、29.39%和3.61%。长三角区域报告的HIV/AIDS患者既存在区域内流动(占比为16.39%),又存在区域内与外流动(占比为18.00%)。多因素Logistic回归分析显示,女性(OR=1.20,95%CI:1.06~1.36)、0~24岁(OR=4.83,95%CI:4.22~5.53)、25~49岁(OR=3.92,95%CI:3.54~4.34),传播途径为同性性传播(OR=1.69,95%CI:1.54~1.85)和其他(OR=1.77,95%CI:1.25~2.51)及报告地为上海市(OR=5.44,95%CI:4.67~6.34)、江苏省(OR=3.60,95%CI:3.14~4.12)、浙江省(OR=4.83,95%CI:4.22~5.53)与HIV/AIDS患者流动性相关。结论 长三角地区的HIV/AIDS患者存在一定的流动性,流动性的特征表现为以省际流动为主,既存在区域内流动,又存在区域内与外流动,且流动与病例的性别、年龄、感染途径等因素有关。Objective To analyze the mobility of HIV/AIDS cases and influencing factors in the Yangtze River Delta(Shanghai, Jiangsu, Zhejiang, and Anhui Province) of China in 2019. Methods As of December 31, 2019, newly diagnosed HIV/AIDS cases in 2019 in the Yangtze River Delta were selected as the study population. We collected basic demographic characteristics, household registration place, reporting place and current address of the study subjects from the HIV/AIDS case reporting information system. Multivariate logistic regression was applied to analyze factors influencing the mobility of HIV/AIDS cases. Results Among the 14048 HIV/AIDS cases reported in the Yangtze River Delta in 2019, 34.39%(4831/14048) had mobility. The mobility reported was the highest(46.58%) in Shanghai and the lowest(12.99%) in Anhui. Interprovincial mobility was predominant among these cases, with 41.18%, 20.33%, 29.39%,and 3.61% in Shanghai, Jiangsu, Zhejiang, and Anhui, respectively. There were both intra-regional flow(16.39%) and extra-regional flow(18.00%) of HIV/AIDS cases. Multivariate logistic regression analysis showed that among the HIV/AIDS cases, the influencing factors for mobility included females(OR=1.20,95%CI: 1.06-1.36), aged 0-24 years(OR=4.83,95%CI: 4.22-5.53) and 25-49 years(OR=3.92, 95%CI: 3.54-4.34), homosexual transmission(OR=1.69, 95%CI: 1.54-1.85)and other transmission modes(OR=1.77, 95%CI: 1.25-2.51), being reported in Shanghai(OR=5.44, 95%CI: 4.67-6.34),Jiangsu(OR=3.60, 95%CI: 3.14-4.12) and Zhejiang(OR=4.83, 95%CI: 4.22-5.53) Provinces. Conclusions There is a certain mobility of HIV/AIDS cases in the Yangtze River Delta and the mobility is characterized as inter-provincial mobility with both intra-regional and extra-regional flows. Mobility was associated with factors such as sex, age, and route of infection.
