广西某地低档场所嫖客HIV感染与安全套使用现况及其相关因素分析

摘　　要：目的 了解广西某地低档场所嫖客人群HIV感染和安全套使用现状及其相关因素,为防控策略制定提供循证依据。方法 2021年6-12月,在广西某地开展横断面调查,在低档娱乐场所通过暗娼推介招募嫖客501人,对其进行面对面调查与HIV检测。利用SAS 9.4软件对数据进行分析整理。结果 501名嫖客的HIV感染率为1.20%(6/501),商业性性行为中未坚持使用安全套的比例为3.99%(20/501),其中50岁及以上老年嫖客HIV感染率为4.62%(3/65),50岁及以上老年嫖客未坚持使用安全套的比例为20.00%(13/65)。经多因素Logistic分析结果显示,嫖客年龄≥50岁(OR=9.01,95%CI:1.50~54.02)、嫖客商业性性行为暴露时间小于一年(OR=24.53,95%CI:2.66~225.77)、商业性性行为活动场所为按摩店、发廊等(OR=12.99,95%CI:1.31~126.30)是嫖客HIV感染的危险因素;嫖客年龄≥50岁(OR=8.88,95%CI:2.98~26.45)、未听说过艾滋病(OR=4.17,95%CI:1.37~12.71)是嫖客未坚持使用安全套的危险因素。结论 广西某地低档场所嫖客中50岁及以上人群HIV感染率较高,商业性性行为不能坚持使用安全套的比例较高,商业性性行为增加HIV感染风险并造成二代传播,应加强对重点场所老年嫖客人群的HIV宣传教育与检测工作。Objective To investigate the current situation and relevant factors associated with HIV infection and condom use among male clients of female sex workers in low-grade entertainment places in Guangxi and to provide evidence for the prevention and control strategies. Methods From June 2021 to December 2021, a cross-sectional survey was conducted in Guangxi. 501 clients were recruited through referrals by female sex workers in low-grade entertainment places. Face-to-face surveys and HIV tests were conducted among recruit people. SAS 9.4 software was used to analyze the data. Results The HIV positive rate of 501 male clients was 1.20%(6/501), and the proportion of not consistently use of condoms in commercial sex behaviors was 3.99%(20/501). The HIV positive rate of elderly clients aged 50 years and above was 4.62%(3/65), and the proportion of not consistently use of condoms was 20.00%(13/65). Multivariate logistic analysis showed that being aged ≥50(OR=9.01,95%CI: 1.50-54.02), being exposed to commercial sexual activities within one year(OR=24.53,95%CI: 2.66-225.77), having commercial sex behavior in massage places and hair salons(OR=12.99,95%CI: 1.31-126.30) were risk factors for HIV infection. Being aged ≥50(OR=8.88,95%CI: 2.98-26.45) and never heard of AIDS(OR=4.17,95%CI: 1.37-12.71) were the risk factors of not consistently use of condoms. Conclusions The HIV positive rate was high among male clients to female sex workers who were aged 50 and above in Guangxi. The proportion of male clients who did not consistently use condoms in commercial sex was high. The risk of HIV infection is high through commercial sex. HIV education and testing should be strengthened in elderly clients to female sex workers in key places.
