摘 要:目的比较多层加压疗法与使用生物冰袋冷敷法在全膝关节置换术后减轻水肿的有效性。方法2020年1月~2022年1月行全膝关节置换手术病人100例。随机分为两组,每组各50例,观察组使用多层加压疗法,对照组使用生物冰袋冷敷法。评估术后1天、2天、3天和7天的肿胀程度,记录肿胀消退时间,评估术后1天和3天的疼痛和舒适度。结果对照组术后1天、3天、5天和7天下肢肿胀处周径分别为(43.51±1.88)cm、(41.97±1.74)cm、(40.60±1.82)cm和(37.93±1.49)cm,观察组下肢肿胀处周径分别为(42.78±1.29)cm、(40.36±1.62)cm、(38.25±1.51)cm和(37.52±1.18)cm,两组比较差异有统计学意义(P<0.05)。观察组术后1天、2天和3天肿胀程度评分明显降低,肿胀消退时间显著减少,差异有统计学意义(P<0.05),术后1天和3天的疼痛评分和舒适度评分比较,差异无统计学意义(P>0.05)。结论多层加压疗法比冰袋冷敷对减轻全膝关节置换术后水肿效果更显著。Objective To compare the effectiveness of multi-layer compression therapy and biological ice pack in alleviating edema after total knee arthroplasty.Methods 100 patients who underwent total knee arthroplasty in our department from January 2020 to January 2022 were selected.They were randomly divided into two groups,50 cases in each group.The observation group was treated with multi-layer compression therapy,while the control group was treated with biological ice pack.The degree of swelling at 1 d,2 d,3 d and 7 d after surgery was evaluated,the time of swelling subtraction was recorded,and the pain and comfort at 1 d and 3 d after surgery were evaluated.Results On postoperative day 1,3,5 and 7,the circumference of lower limb swelling in the control group was(43.51±1.88)cm、(41.97±1.74)cm、(40.60±1.82)cm and(37.93±1.49)cm,respectively.The circumference of lower limb swelling in the observation group was(42.78±1.29)cm,(40.36±1.62)cm,(38.25±1.51)cm and(37.52±1.18)cm,respectively,which were significantly lower than those in the control group(P<0.05).Besides,the swelling degree scores were significantly reduced at 1 d,2 d and 3 d(P<0.05),and the swelling regression time was reduced compared to the control group(P<0.05),while the pain scores and comfort scores of observation group were not significantly different at 1 d and 3 d after operation(P>0.05).Conclusion Multi-layer compression therapy was more effective than ice pack cold compress in alleviating edema after total knee arthroplasty.
