摘 要:目的 了解2017—2020年广东省惠州市医疗机构和托幼机构消毒质量现状,为加强其消毒管理工作,并防止其传染病发生及流行提供合理依据。方法 对惠州市不同级别的医疗机构和托幼机构两类重点机构分层抽样监测,以描述流行病学方法分析合格率等监测结果,合格标准按《2013年广东省医疗、托幼机构消毒质量监测技术方案(试行)》中相关要求。结果 2017—2020年两类重点机构共检查样本12 672份,合格11 822份、合格率93.29%,不同年度医疗机构(趋势χ^(2)=10.623)和托幼机构(趋势χ^(2)=9.941)监测覆盖率均呈逐年下降趋势(P<0.05);医疗机构共检查样本10 601份,合格10 127份、合格率95.53%,合格率2017年最高(97.46%)、2019年最低(合格率93.20%),不同年度合格率差异有统计学意义(χ^(2)=64.566,P<0.05);托幼机构共检查样本2071份,合格1695份、合格率81.84%,合格率2017年最高(90.98%)、2019年最低(78.31%),不同年度合格率差异有统计学意义(χ^(2)=34.840,P<0.05);合格率县级医院最高(98.09%)、个体诊所最低(92.45%),不同级别医疗机构消毒质量合格率差异有统计学意义(χ^(2)=31.445,P<0.05);托幼机构合格率幼儿人数在50以下最高(85.71%)、50~199人的托幼机构最低(75.69%),不同级别托幼机构消毒质量合格率差异有统计学意义(χ^(2)=47.112,P<0.05)。结论 惠州市医疗机构消毒质量良好,托幼机构消毒质量较差;应重点管理乡(镇)卫生院及个体诊所及人数较多的托幼机构,指导其机构做好消毒工作,避免传染病的暴发及流行。Objective To understand the current situation of disinfection quality in medical institutions and nurseries in Huizhou city of Guangdong province, so as to provide reasonable basis for strengthening disinfection management and preventing the occurrence and prevalence of infectious diseases. Methods From 2017 to 2020, medical institutions and nurseries of different levels in Huizhou were monitored by stratified sampling, and the monitoring results were analyzed by descriptive epidemiological method. The eligibility standard was referred to the Technical Program of Disinfection Quality Monitoring of Medical and Nurseries in Guangdong Province in 2013(Trial). Results Totally 12 672 samples were examined in two types of important institutions from 2017 to 2020, of which 11 822 samples were qualified, with a qualified rate of 93.29%. The monitoring coverage rates of medical institutions(χ^(2)=10.623) and nursing institutions(χ^(2)=9.941) showed decreasing trend year by year(P<0.05). A total of 10 601 samples were detected in medical institutions, of which 10 127samples were qualified, with a qualified rate of 95.53%. The qualified rate was the highest in 2017(97.46%) and the lowest in 2019(93.20%), and the difference of qualified rates in different years was statistical(χ^(2)=64.566, P<0.05). There were 2 071samples examined in nurseries, among which 1 695 samples were qualified and the qualified rate was 81.84%. The qualified rate was the highest in 2017(90.98%) and the lowest in 2019(78.31%), and there was statistical difference among different years(χ^(2)=34.840, P<0.05). The qualified rate was the highest in county-level hospitals(98.09%) and the lowest in individual clinics(92.45%). The difference of qualified rates among different levels of medical institutions was statistical significant(χ^(2)=31.445, P<0.05).
