6例血栓性血小板减少性紫癜的诊疗分析

摘　　要：目的 探讨血栓性血小板减少性紫癜(TTP)的临床表现及诊疗经验。方法 对三门峡市中心医院2015年—2021年,收治的6例血栓性血小板减少性紫癜患者的临床资料进行回顾性分析。结果 6例患者中3例女性,3例男性,年龄为12岁~73岁,其中1例伴妊娠发病,1例儿童发病,4例成年人无诱因发病。1例治愈后复发,1例死亡,其余4例治愈出院。6例血栓性血小板减少性紫癜患者的诊疗中,共进行血浆置换48次。实验室检查的血色素(Hb)、血小板计数(Plat)、乳酸脱氢酶(LDH)、破碎红细胞百分比,在疾病治疗前后变化明显,具有疗效评估价值。结论 重视血栓性血小板减少性紫癜的早期诊断,及时进行血浆置换,降低输血不良反应风险,治疗效果及预后较好。Objective This paper aims to investigate the clinical features and the treatment experience of thrombotic thrombocytopenic purpura(TTP). Methods The clinical data of 6 patients with thrombotic thrombocytopenic purpura treated in Sanmenxia Central Hospital from 2015 to 2020 were retrospectively analyzed. Results Among the 6 patients, 3 were female and 3 were male, aged from 12 to 73 years. Among them, 1 case developed the disease with pregnancy, 1 case developed the disease with childhood, and 4 adults developed the disease without inducing factors. One case relapsed after cure, one died, and the other 4 cases were cured and discharged. Plasma exchange was performed 48 times in 6 patients with thrombotic thrombocytopenic purpura. Hemoglobin(Hb), platelet count(Plat), lactate dehydrogenase(LDH) and percentage of broken red blood cells in laboratory tests showed significant changes before and after treatment, which had curative effect evaluation value. Conclu sion Attention should be paid to early diagnosis of thrombotic thrombocytopenic purpura and timely plasmapheresis to reduce the risk of transfusion adverse reactions, so as to achieve good therapeutic effect and prognosis.
