摘 要:目的:分析大螺距双期模式下行头颈和冠状动脉电子计算机断层扫描血管造影(CTA)技术在心率波动患者中的应用价值。方法:选取在医院影像科检查的122例心率波动患者临床资料,根据检查方法的不同将其分为大螺距扫描组(行大螺距双期模式头颈心CTA一站式扫描)和常规扫描组(行头颈CTA减影扫描和冠状动脉CTA扫描),每组61例。比较两组CTA检查图像的主观评分、头颈CTA检查图像的客观评价指标、冠状动脉CTA检查图像的客观评价指标,以及辐射剂量与对比剂用量。结果:两组CTA检查图像的主观评分差异无统计学差异。常规扫描组头颈CTA检查图像信噪比(SNR)、对比噪声比(CNR)和标准偏差(SD)指标均高于大螺距扫描组,差异有统计学意义(t=5.138,t=13.613,t=15.888;P<0.05)。常规扫描组冠状动脉CTA检查图像的CNR、SD高于大螺距扫描组,差异有统计学意义(t=13.903,t=7.821;P<0.05)。大螺距扫描组对比剂用量、有效辐射剂量(ED)及剂量长度乘积(DLP)低于常规扫描组,差异有统计学意义(t=56.296,t=15.575,t=29.132;P<0.05)。结论:大螺距双期模式下行头颈和冠状动脉CTA扫描在心率波动患者中具有应用可行性,而较低的辐射剂量与对比剂用量即可得到满意的图像质量。Objective: To analyze the application value of computed tomography angiography(CTA) technique under double-phase mode with large screw pitch on head and neck and coronary artery of patients with heart rate fluctuation.Methods: The clinical data of 122 patients with heart rate fluctuation who admitted to the imaging department of hospital were selected. According to different examination methods, they were divided into the large screw pitch scan group that underwent CTA one-stop scan on heat and neck and heart by double-phase mode with large screw pitch and the conventional scan group that respectively underwent CTA subtraction scan on head and neck and CTA scan on coronary artery, with 61 cases in each group. Subjective score of CTA examination images, objective evaluation index of CTA examination images on head and neck, objective evaluation index of CTA examination images on coronary artery, the radiation dose and the dosage of contrast agent were compared between the two groups. Results: There was no statistically significant difference in the subjective scores of CTA images between the two groups. The SNR, CNR and standard deviation(SD) of CTA examined images of head and neck of conventional scan group were significantly higher than those of large screw pitch scan group(t=5.138, t=13.613, t=15.888, P<0.05), respectively. The CNR and SD of CTA examined images of coronary artery of conventional group were significantly higher than those of large screw pitch scan group(t=13.903, t=7.821, P<0.05), respectively. The dosage of contrast agent, the effective radiation dose(ED) and dose-length product(DLP) of the large screw pitch scan group were significantly lower than those of the conventional scan group(t=56.296, t=15.575, t=29.13, P<0.05), respectively. Conclusion: CTA scan has feasibility for head and neck and coronary artery of patients with heart rate fluctuation under double-phase mode with large screw pitch, while the lower radiation dosage and the dosage of contrast agent can obtain satisfying image quality
