摘 要:目的 探讨职业噪声接触工人双耳高频平均听阈与高血压的相关性。方法 利用常州市武进区职业噪声接触在岗职工健康体检数据,采用多因素logistics回归模型,探讨双耳高频平均听阈与高血压的相关性。结果 2020年常州市武进区共46 545名噪声作业在岗工人纳入研究,平均年龄(39.90±10.20)岁,其中高频听力轻度损失9 418人(占20.23%),高频听力重度损失4 738人(占10.18%)。正常听力、高频听力轻度损失、高频听力重度损失3组高血压患病率分别为26.37%、33.30%、35.99%。多元logistics回归结果显示,与正常听力组相比,高频听力轻度损失和重度损失组的高血压患病风险逐渐增加,OR(95%CI)分别为1.31 (1.25~1.38)和1.44 (1.35~1.54),其中男性分别为1.31 (1.24~1.39)和1.47 (1.37~1.58),女性分别为1.33 (1.19~1.49)和1.27 (1.08~1.50)。结论 职业噪声接触工人双耳平均高频听阈与高血压存在相关性,应关注高频听力损失工人的血压状况。Objective To investigate the correlation between high-frequency mean hearing threshold and hypertension among workers exposed to occupational noise. Methods The physical examination data of all the workers exposed to occupational noise were collected, the multivariate logistic regression models were used to explore the correlation between binaural high frequency mean auditory threshold and hypertension. Results A total of 46 545 workers exposed to occupational noise were recruited in the study, with an average age of 39.90±10.20 years.Among all subjects, 9 418(accounted for 20.23%) had high-frequency mild hearing loss and 4 738 had high-frequency severe hearing loss(accounted for 10.18%).The prevalence of hypertension in the normal-hearing group, mild high-frequency hearing loss group and severe high-frequency hearing loss group were 26.37%,33.30% and 35.99%,respectively.The multivariate logistic regression analysis showed that the risk of hypertension was gradually increased in the group with mild high-frequency hearing loss and severe hearing loss compared with the normal-hearing group, with OR(95%CI) of 1.31(1.25-1.38) and 1.44(1.35-1.54),respectively;for male and female subjects, the OR(95%CI) values were 1.31(1.24-1.39),1.47(1.37-1.58) and 1.33(1.19-1.49),1.27(1.08-1.50),respectively. Conclusion There is correlation between high-frequency mean hearing threshold and hypertension among workers exposed to occupational noise.More attention should be paid to monitor blood pressure status of workers with high-frequency hearing loss.
