摘　　要：目的:探讨硅胶假体联合脱细胞异体真皮基质(Acellularallogeneicdermalmatrix,AADM)在鼻整形术中的效果及满意度情况。方法:选取2017年9月-2019年9月在医院行鼻整形的91例就医者为研究对象,均采用硅胶假体联合AADM进行鼻整形术。术后对其随访6~12个月,平均随访(8.90±1.24)个月,记录就医者临床效果,包括鼻长度和鼻尖高度变化、满意度及并发症情况。结果:术后,整形效果优者55例(55/91,60.44%),良29例(29/91,31.87%),一般6例(6/91,6.59%),差1例(1/91,1.10%),优良率为92.31%(84/91)。术后就医者鼻长度长于术前,鼻尖高度高于术前(P<0.05);在就医者自我评价中,非常满意49例(49/91,53.85%),满意36例(36/91,39.56%),不满意6例(6/91,6.59%),总满意度为93.41%(85/91);术后对就医者随访6~12个月,平均随访(8.90±1.24)个月,就医者均无感染、鼻部瘀血肿胀、假体移位、假体轮廓外显、皮肤破溃等严重并发症;5例就医者出现轻度鼻尖发红,经热敷处理后症状消失。结论:硅胶假体联合AADM在鼻整形术中具有良好的修复效果,可有效改善鼻形,且并发症少,就医者满意度高,值得临床推广应用。Objective To investigate the effect of silicone prosthesis combined with acellular allogeneic dermal matrix(AADM) in rhinoplasty and the satisfaction of patients. Methods Total of 91 patients who underwent rhinoplasty in our hospital from September 2017 to September 2019 were selected.All patients were treated with silicone prosthesis combined with AADM for rhinoplasty. The patients were followed up for 6-12 months after the operation,with an average follow-up of(8.90±1.24) months.The patient’s clinical effects,changes in the length of the nose and the height of the nose tip,satisfaction,and complications were recorded. Results After the operation,plastic surgery results showed the that 55 cases were excellent(55/91,60.44%),29 cases were good(29/91,31.87%),6 cases were general(6/91,6.59%),and 1 case was poor(1/91,1.10%),the excellent and good rate was 92.31%(84/91). The length of nose of patients after operation was longer than that before operation, and the height of nasal tip was higher than that before operation(P<0.05). After the operation,in that self-evaluation of the patient,49 patients were very satisfied(49/91,53.85%),36 patients were satisfied(36/91,39.56%),and 6 patients were dissatisfied(6/91,6.59%).The total satisfaction rate was 93.41%(85/91). The patients were followed up for 6-12 months after the operation,with an average follow-up of(8.90±1.24) months.There were no serious complications such as infection,nasal congestion and swelling,prosthesis displacement,prosthesis contour appearance,and nasal tip skin ulceration;5 patients had mild nasal redness,and the symptoms disappeared after hot compress. Conclusion Silicone prosthesis combined with AADM has a good repair effect in rhinoplasty,which can effectively improve the shape of the nose,with fewer complications,and the satisfaction of patients was high.It is worthy of clinical application.
