Effects of Exercise Intervention on Glucose and Lipid Metabolism in the Prediabetes Population with Different Phenotypes

摘  要:目的:确定糖尿病前期的表型是否会影响有氧运动对其血糖控制、体型及心血管风险因素的改善程度,旨在为糖尿病前期人群提供个性化的运动处方建议,逆转或延缓其发展为糖尿病。方法:通过对32名年龄较大(58.22±2.76岁)、超重的糖尿病前期患者(BMI:28.61±2.64kg/m 2)进行12周有氧运动干预,于干预前和干预后评估患者的血糖水平(空腹血糖、2小时口服葡萄糖耐量、糖化血红蛋白);体型(体重、身体质量指数、腰围)和心血管健康指标(总胆固醇、收缩压、舒张压、高密度脂蛋白、低密度脂蛋白),比较三种不同表型患者的改善程度。结果:干预前后,糖尿病前期人群总体的空腹血糖、糖化血红蛋白、体型(体重、身体质量指数、腰围)、心血管健康指标(总胆固醇、收缩压、舒张压、高密度脂蛋白、低密度脂蛋白)均有非常显著的改善(P<0.01),达到改善葡萄糖稳态、减轻肥胖和降低心血管风险的目的,而餐后2小时糖耐量未见明显改善(P>0.05),并且空腹血糖的显著改善仅出现在单纯空腹血糖受损、空腹血糖合并糖耐量受损的患者中,而餐后2小时糖耐量的改善在三种表型糖尿病前期患者中均未出现。结论:无饮食干预的中等强度有氧运动对糖尿病前期人群的体型和血脂健康均有显著改善,其改善程度在三种不同表型患者中未见明显差异,而其对空腹血糖的改善效果仅出现于单纯空腹血糖受损、空腹血糖受损合并糖耐量受损的患者中,并且其对三种表型糖尿病前期人群的餐后2小时血糖均无显著改善作用。The objective of this paper is to determine whether the phenotype of prediabetes affects the degree to which aerobic exercise can improve blood glucose control,body shape and cardiovascular risk factors,so as to provide personalized exercise prescription recommendations for prediabetes and reverse or delay the development of diabetes.Methods:32 older(58.22±2.76 years old)and overweight(BMI:28.61±2.64kg/m 2)patients with prediabetes were treated with aerobic exercise for 12 weeks.The blood glucose levels(fasting blood glucose,2-hour oral glucose tolerance,HBA1c)were evaluated before and after the intervention.Body shape(weight,body mass index,waist circumference)and cardiovascular health indicators(total cholesterol,systolic blood pressure,diastolic blood pressure,high-density lipoprotein,low-density lipoprotein)were compared for the extent of improvement in patients with three different phenotypes.Results:Before and after the intervention,fasting blood glucose,glycosylated hemoglobin,body shape(body weight,body mass index,waist circumference),cardiovascular health indicators(total cholesterol,systolic blood pressure,diastolic blood pressure,high density lipoprotein,low density lipoprotein)were significantly improved in the prediabetic population(P<0.01)to improve glucose homeostasis,reduce obesity and reduce cardiovascular risk,but no significant improvement was found in glucose tolerance 2 hours after meals(P>0.05),and the significant improvement of fasting blood glucose was only found in patients with impaired fasting blood glucose alone or with impaired glucose tolerance.However,the improvement of glucose tolerance 2 hours after meal was not seen in the three phenotypic prediabetics.Conclusions:Moderate intensity aerobic exercise without dietary intervention can significantly improve the body shape and blood lipid health of the prediabetic group,and the degree of improvement is not significantly different among the patients with three different phenotypes.However,the improvement effect of moderate intensity
