儿童睾丸附件扭转和睾丸扭转的超声表现及分析

摘　　要：目的探讨超声在儿童睾丸扭转和睾丸附件扭转鉴别和诊断中的应用。方法分析111例睾丸附件扭转和13例睾丸扭转的二维及彩色多普勒超声声像图特点,总结两者鉴别要点。结果睾丸附件扭转超声表现为附睾头部及阴囊之间高回声结节,伴有附睾体积增大,鞘膜腔积液。睾丸扭转超声表现为患侧睾丸回声不均匀,出现显示散在片状低回声。彩色多普勒显示扭转的睾丸呈缺血改变。结论超声检查在儿童睾丸附件扭转和睾丸扭转的鉴别及诊断中具有一定的价值。Objective To explore the application of ultrasonography in the differential diagnosis of testicular torsion and testicular appendage torsion in children.Methods Two-dimensional and color Doppler ultrasonographic features of 111 cases of testicular appendage torsion and 13 cases of testicular torsion were analyzed,and the main points of differential diagnosis were summarized.Results Ultrasonography of testicular appendage torsion showed hyperechoic nodules between the head of epididymis and scrotum,accompanied by increased epididymis volume and hydrocele of tunica vaginalis.Ultrasonography of testicular torsion showed that the testicular echo of the affected side was uneven,showing scattered patchy low echo.Color Doppler showed ischemic changes in the twisted testis.Conclusion Ultrasonography is valuable in the differential diagnosis of testicular appendage torsion and testicular torsion in children.
