摘 要:目的研究高尿酸与体重指数(BMI)交互作用对苗族人群代谢综合征的影响。方法采用多阶段分层整群抽样方法,在贵州省30~79岁苗族居民中进行调查,共纳入研究对象共4672人。Logistic回归模型分析高尿酸、BMI与代谢综合征的关系,通过计算交互作用的相对超额危险度比(RERI)、归因比(AP)、协同指数(S)、纯因子归因交互作用百分比(AP×AB)%,来评估相加交互作用。结果本研究中MS的检出率为20.17%、高尿酸的检出率为25.09%、BMI≥24检出率为59.85%。在调整了潜在混杂因素后,与非高尿酸者相比、高尿酸者患MS风险的OR=2.640,95%CI:2.251~3.097(P<0.001)。BMI≥24与MS的患病风险相关(OR=8.430,95%CI:6.738~10.560,P<0.001),但二者对MS无相乘交互作用(OR=0.754,95%CI:0.476~1.194,P=0.228)。在相加交互作用分析中,高尿酸合并BMI≥24者的患病风险OR=18.976,95%CI:13.995~25.730(P<0.001),高于只有高尿酸或者只有BMI≥24者,高尿酸与BMI≥24对MS的患病风险有相加交互作用,RERI(95%CI)、AP(95%CI)、S(95%CI)为8.321(4.800~11.843)、0.439(0.336~0.542)、1.862(1.525~2.274)、AP×AB(%)为46.29%。结论高尿酸和体重指数都与代谢综合征的风险增加有关,高尿酸与体重指数对MS的患病风险存在相加交互作用。Objective To analyze the interaction of hyperuricemia and BMI on the risk of metabolic syndrome in the Miao population.Methods The multistage stratified cluster sampling method was conducted among Miao residents aged 30 to 79 years in Guizhou Province,a total of 4672 study subjects were included.The Logistic regression model was used to analyze the relationship between hyperuricemia and BMI on metabolic syndrome risk,additive interaction was evaluated by RERI,AP,S,(AP×AB)%.Results In this study,the detection rate of MS was 20.17%,hyperuricemia was 25.09%,and BMI≥24 was 59.85%.After adjusting for potential confounders,the MS risk of hyperuricemia patients OR=2.640,95%CI:2.25-3.097(P<0.001)compared with non-hyperuricemia patients.BMI≥24 was associated with the disease risk of MS(OR=8.430,95%CI:6.738-10.560,P<0.001).There was not multiplicative interaction between hyperuricemia and BMI with MS(OR=0.754,95%CI:0.476-1.194,P=0.228).In the additive interaction analysis,patients with hyperuricemia plus BMI≥24 had a higher risk(OR=18.976,95%CI:13.995-25.730,P<0.001)than those with hyperuricemia only or BMI≥24 alone.There was additive interaction between hyperuricemia and BMI on the risk of MS,RERI(95%CI),AP(95%CI),and S(95%CI)were 8.321(4.800-11.843),0.439(0.336-0.542),1.862(1.525-2.274),AP×AB(%)was 46.29%.Conclusion Both hyperuricemia and BMI were related to a high risk of MS.There is additive interaction between hyperuricemia and BMI on the risk of MS.
