小瞳孔下连续环形撕囊技术在超声乳化白内障术中的应用

摘　　要：目的 探讨超声乳化白内障术中应用小瞳孔下连续环形撕囊技术的疗效。方法 17例(21眼)小瞳孔白内障患者接受白内障超声乳化手术治疗,术中采用小瞳孔下连续环形撕囊技术,观察术中及术后并发症的发生等情况。结果 17例(21眼)采用小瞳孔下连续环形撕囊技术,术中出现囊袋口放射状撕裂1眼(4.76%),囊带口欠规则略呈梅花状2眼(9.52%),术后出现角膜水肿6眼(28.57%),术后出现眼压升高者3眼(14.29%),术后出现角膜后KP、房水混浊等前房炎症性反应2眼(9.52%),无后囊膜破裂、悬韧带断离、前房出血等并发症出现。结论 在未采用瞳孔扩大装置的小瞳孔白内障超声乳化术中,小瞳孔下连续环形撕囊技术是一种能够有效完成术中连续环形撕囊的手术技巧。Objective Discuss the effect that adopt continuous circular capsulorhexis with small pupil in phacoemulsification cataract surgery.Method Seventeen patients(21 eyes) with small pupil cataract underwent phacoemulsification.Intraoperative use of small pupil continuous curvilinear capsulorhexis method to observe the occurrence of intraoperative and postoperative complications Result Seventeen patients(21 eyes) were treated with small pupil continuous curvilinear capsulorhexis.In the operation,one eye(4.76%) of the radial tear of the capsular bag was found,and two eyes(9.52%) of the irregular capsular is plum flower-like were observed.Comeal edema in six eyes(28.57%),3 eyes(14.29%) with increased intraocular pressure after ope rati on,postoperative anterior chamber inflammatory reactions such as KP and turbid aqueous humor occurred in 2 eyes(9.52%),no complications such as posterior capsule rupture,suspensory ligament detachment,anterior chamber hemorrhage.Conclusion Small pupil continuous curvilinear capsulorhexis is a surgical technique that can effectively complete intraoperative continuous circular capsulorhexis in phacoemulsification of small pupil cataract without pupil dilation device.
