摘　　要：目的观察铍针联合CT引导下脊神经后支脉冲射频消融治疗腰椎骨关节病的临床效果。方法选取2017年5月至2020年5月在首都医科大学附属北京中医医院骨伤科运用铍针联合CT引导下脊神经后支脉冲射频消融治疗腰椎骨关节病的50例患者。所有患者术后随访1年,记录治疗前后视觉模拟量表(VAS)评分,采用改良MacNab标准评价患者治疗后3个月、1年的临床疗效。结果患者治疗后24 h、1周、1个月、3个月、1年VAS评分均低于术前[(2.24±0.94)、(2.00±0.86)、(1.80±0.40)、(1.34±1.30)、(2.14±1.95)分比(6.16±0.77)分],差异均有统计学意义(均P<0.05)。治疗后3个月时患者优良率为84.0%(42/50),1年时患者优良率为72.0%(36/50)。患者术后穿刺部位均未见出血、感染、神经损伤等并发症发生。结论铍针联合CT引导下脊神经后支脉冲射频消融治疗腰椎骨关节病安全有效。Objective To observe the clinical effect of beryllium needle combined with CT-guided pulsed radiofrequency ablation of spinal nerve posterior branch on the treatment of lumbar vertebra osteoarthropathy.Methods From May 2017 to May 2020,50 patients with lumbar vertebra osteoarthropathy treated with beryllium needle combined with CT-guided pulsed radiofrequency ablation of spinal nerve posterior branch were enrolled from Department of Orthopedics,Beijing Hospital of Traditional Chinese Medicine,Capital Medical University.All the patients were followed-up for 1 year.The visual analog scale(VAS)score was recorded before and after treatment,and modified MacNab standard was used to evaluate the clinical effect at 3 months and 1 year after treatment.Results The VAS scores at 24 h,1 week,1 month,3 months and 1 year after treatment were lower than those before treatment[(2.24±0.94),(2.00±0.86),(1.80±0.40),(1.34±1.30),(2.14±1.95)vs(6.16±0.77)],and the differences were statistically significant(all P<0.05).The excellent-good rate was 84.0%(42/50)at 3 months after treatment,and 72.0%(36/50)at 1 year after treatment.No bleeding,infection,nerve injury and other complications occurred at puncture site after operation.Conclusion Beryllium needle combined with CT-guided pulsed radiofrequency ablation is safe and effective on the treatment of lumbar vertebra osteoarthropathy.
