摘　　要：目的探讨Bryan人工颈椎间盘角对颈椎病疗效的影响。方法选择2016年1月~2018年1月本院收治的90例颈椎病患者,均采用Bryan人工颈椎间盘置换(cervical artificial disc replacement,CADR)治疗,术后7d测量Bryan人工颈椎间盘角,负值设为后凸组,正值设为正常组,分别为32例、58例。比较两组末次随访时的节段活动度、颈椎功能障碍指数(neck disability index,NDI)、颈椎总活动度、JOA评分等功能及影像指标,评价椎旁骨化(paravertebral ossification,PO)分级。结果90例颈椎病患者术后Bryan人工颈椎间盘角为负值者32例,正值者58例,Bryan人工颈椎间盘角后凸率35.56%;两组颈椎总活动度差异无统计学意义(P>0.05),但后凸组的节段活动度显著低于正常组(P<0.05);两组NDI指数、JOA评分差异无统计学意义(P>0.05);后凸组PO分级较正常组更为严重(P<0.05)。结论Bryan CADR手术治疗颈椎病后,易发生Bryan人工颈椎间盘角后凸,且影响节段活动度,容易加重椎旁骨化。Objective To explore the effect of Bryan artificial cervical disc angle on the curative effect of cervical spondylosis.Methods Ninety patients with cervical spondylosis in our hospital from January 2016 to January 2018 were selected,all of whom were treated with Bryan cervical artificial disc replacement(CADR).The Bryan artificial cervical disc angle was measured 7 days after the operation.The negative value was set as the kyphosis group,and the positive value was set as the normal group.There were 32 cases and 58 cases respectively.The functional and imaging indexes such as segmental mobility,cervical spine dysfunction index(NDI),total cervical spine mobility,and JOA score at the last follow-up between the two groups were compared.The classification of paravertebral ossification(PO)were evaluated.Results After 90 patients with cervical spondylosis,there were 32 cases with negative Bryan artificial cervical disc angle and 58 cases with positive value.The kyphosis rate of Bryan artificial cervical disc angle was 35.56%;there was no significant difference in the total range of motion between the kyphosis group and the normal group(P>0.05).The segment activity was significantly lower than the control group(P<0.05).There was no statistically significant difference in NDI index and JOA score between the kyphosis group and the normal group(P>0.05).The PO classification of the kyphosis group was more serious than that of the normal group(P<0.05).Conclusion After Bryan CADR treatment of cervical spondylosis,Bryan artificial cervical disc angle kyphosis is prone to occur,and it affects the segmental mobility and easily aggravates the paravertebral ossification.
