益气活血通脉汤联合推拿治疗闭合性Pilon骨折临床研究

摘　　要：目的:观察益气活血通脉汤联合推拿治疗闭合性Pilon骨折的临床疗效。方法:选取闭合性Pilon骨折患者190例,按随机数字表法分为对照组和观察组各95例。对照组给予20%甘露醇注射液静脉滴注,观察组给予益气活血通脉汤联合推拿治疗。比较2组肿胀治疗效果及骨折愈合情况。结果:观察组总有效率为87.37%,高于对照组73.68%(P<0.05)。观察组小腿周径变化时间、入院至手术时间均短于对照组(P<0.05)。观察组软组织修复时间、肿胀消除时间、骨折愈合时间均短于对照组(P<0.05)。观察组术前、术后1 d、术后7 d疼痛评分均低于对照组(P<0.05)。观察组术后随访3个月、6个月CT值高于对照组(P<0.05)。结论:益气活血通脉汤联合推拿能显著缓解闭合性Pilon骨折患者早期肢体肿胀,在短时间内消除肿胀,有效减轻患肢疼痛,缩短手术准备时间,为患者预后创造有利条件。Objective:To observe the clinical effect of the therapy of Yiqi Huoxue Tongmai Decoction combined with Chinese massage on closed Pilon fracture.Methods:A total of 190 patients with closed pilon fracture were selected and divided into the control group and the observation group according to random number table method,with 95 patients in each group.The control group was given 20% Mannitol Injection by intravenous drip,and the observation group was given the therapy of Yiqi Huoxue Tongmai Decoction combined with Chinese massage.The therapeutic effect on swelling and the fracture healing were compared between the two groups.Results:The total effective rate was 87.37% in the observation group,higher than that of 73.68% in the control group(P<0.05).The change time of calf circumference diameter and the time from admission to surgery in the observation group were significantly shorter than those in the control group(P<0.05).The time of soft tissue repair,swelling elimination and fracture healing in the observation group was respectively shorter than that in the control group(P<0.05).The pain scores in the observation group before surgery,one day after surgery and 7 days after surgery were lower than those in the control group(P<0.05).The CT value in the observation group was higher than that in the control group at the third month and sixth month of follow-up after the surgery(P<0.05).Conclusion:The therapy of Yiqi Huoxue Tongmai Decoction combined with Chinese massage can significantly relieve the early limb swelling of patients with closed Pilon fracture,eliminate the swelling in a shorter time,effectively reduce the pain of affected limbs,shorten the preparation time for surgery,and create favorable conditions for the prognosis.
