活络骨康丸治疗老年骨质疏松性髋部骨折内固定术后临床研究

摘　　要：目的:观察活络骨康丸对老年骨质疏松性髋部骨折内固定术后患者骨代谢指标及骨密度(BMD)的影响。方法:将102例老年骨质疏松性髋部骨折患者随机分为观察组52例与对照组50例,2组均在术后实施常规治疗,观察组另予活络骨康丸治疗。比较2组治疗前后骨代谢指标及BMD水平。结果:治疗前,2组血清骨特异性碱性磷酸酶(BALP)、骨钙素(BGP)、Ⅰ型原胶原C-端前肽(PICP)、Ⅰ型原胶原N-端前肽(PINP)、抗酒石酸酸性磷酸酶5b (TRACP5b)、Ⅰ型胶原交联C-末端肽(S-CTX)水平比较,差异无统计学意义(P>0.05)。治疗后,2组血清BALP、BGP、PICP、PINP水平均升高(P<0.05),且观察组高于对照组(P<0.05);2组血清TRACP5b、S-CTX水平均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗前,2组胸椎、腰椎BMD水平比较,差异无统计学意义(P>0.05);治疗后,2组胸椎、腰椎BMD水平均升高(P<0.05),且观察组高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:活络骨康丸可促进老年骨质疏松性髋部骨折患者内固定术后骨形成,减少骨吸收,增加BMD,且安全性高。Objective:To observe the effect of Huoluo Gukang Pills on bone metabolism indexes and bone mineral density(BMD) in elderly patients with osteoporotic hip fractures after internal fixation.Methods:A total of 102 cases of elderly patients with osteoporotic hip fractures were randomly divided into the observation group and the control group,with 52 and 50 cases in each group respectively. Both groups were given routine treatment after the surgery, and the observation group was additionally treated with Huoluo Gukang Pills. The bone metabolism indexes and BMD were compared before and after treatment in the groups. Results:Before treatment,there was no significant difference being found in the comparison of bone alkaline phosphatase(BALP), bone gla protein(BGP), procollagen type I carboxy-terminal propeptide(PICP),procollagen type I N-terminal propeptide(PINP),tartrate-resistant acid phosphatase 5b(TRACP5b), carboxy-terminal collagen crosslinks(S-CTX) in serum between the two groups(P>0.05).After treatment,the levels of BALP,BGP,PICP,and PINP in serum in the two groups were increased(P<0.05),and the four levels in the observation group were higher than those in the control group(P<0.05);the levels of TRACP5b and S-CTX in serum in the two groups were decreased(P<0.05),and the two levels in the observation group were lower than those in the control group(P<0.05). Before treatment,there was no significant difference being found in the comparison of the levels of BMD of thoracic and lumbar vertebrae between the two groups(P>0.05). After treatment,the levels of BMD of thoracic and lumbar vertebrae in the two groups were increased(P<0.05),and the two levels in the observation group were higher than those in the control group(P<0.05). There was no significant difference being found in the comparison of the incidence of adverse reactions between the two groups(P>0.05). Conclusion: For elderly patients with osteoporotic hip fractures after internal fixation, Huoluo Gukang Pills can promote bone formation,reduce bone absorption
