摘　　要：1例38岁女性患者,因“四肢皮肤出血点20 d,血小板减低2周”入院。临床药师首次药学查房评估患者基本情况以及院外药物治疗方案,加强患者教育,提供药物咨询,参与院内药物治疗全过程,采用药物治疗管理服务理念,从药物治疗的安全性、有效性和适应证方面评估药物治疗方案的合理性,深入分析药物治疗相关问题,及时发现药品不良反应并干预,密切监护患者病情变化,查阅相关资料和文献,并为临床合理用药提供有价值的参考。患者治疗后随访良好。A 38-year-old female patient was admitted because of 20 days of limb skin blood points and 2 weeks of thrombocytopenia.Clinical pharmacists assessed her basic situation and the out-of-hospital drugs in the first pharmaceutical ward round,strengthened patient education,provided drug consultation,participated in the whole process of in-hospital drug treatment,evaluated the safety,effectiveness and indications of drugs based on the concept of drug treatment management service,deeply analyzed the problems related to drugs,timely found and dealt with adverse drug reactions,closely monitored the changes of her condition,reviewed relevant data and literature,and provided valuable references for clinical rational drug use.The patient was well in the follow-up after treatment.
