我院儿科住院患者糖皮质激素的应用现状调查与分析

摘　　要：目的:了解我院儿科糖皮质激素(GCs)的应用现状,为规范其临床应用提供参考。方法:从医院信息系统检索2021年1月–2022年3月我院儿科使用GCs的出院患者病历信息,统计患者基本情况、临床诊断、单次用药剂量、总用药剂量、总疗程等信息,对医嘱进行回顾性分析及合理性评价。结果:共计1137例患者使用了GCs,6岁以下患者1031例(90.68%)。734例(64.56%)患者使用1种GCs,401例(35.27%)患者使用2种GCs。1137例患者共使用GCs 1542例次,其中吸入用布地奈德混悬液使用次数最多(68.74%);256例(22.51%)患者存在不合理用药问题,表现为无适应证用药(137例,12.05%)和遴选药品不适宜(119例,10.47%)。2例(0.18%)患者出现药品不良反应。结论:我院儿科GCs尚存在不合理用药现象,需加强管理,以保证临床用药的安全性和有效性。Objective:To investigate the clinical application of glucocorticoids(GCs)in pediatric inpatients of our hospital,so as to provide references for rational drug use.Methods:Medical records of discharged pediatric inpatients using GCs in our hospital from January 2021 to March 2022 were collected from the hospital information system,and were retrospectively analyzed and rationally evaluated in terms of general information,clinical diagnosis,single dosage,total dosage and total course of the treatment,etc.Results:A total of 1137 pediatric inpatients used GCs.There were 1031 pediatric inpatients under 6 years old(90.68%),734 cases(64.56%)using one GCs,and 401 cases(35.27%)using two GCs.All the pediatric inpatients used GCs 1542 times,among which inhaled budesonide suspension was the top 1(68.74%).There were 256 cases(22.51%)using GCs irrationally,among which 137 cases(12.05%)had no indication,119 cases(10.47%)used improper GCs.There were two cases(0.18%)developing adverse drug reactions.Conclusion:There was irrational use of GCs in pediatric inpatients of our hospital,which needed further management to insure the safety and efficacy use of GCs.
