清热消痈汤内服结合消炎散外敷治疗早期肛周脓肿疗效及安全性观察

摘　　要：目的:观察清热消痈汤内服结合消炎散外敷治疗早期肛周脓肿的疗效及安全性。方法:将肛周皮下脓肿患者80例随机分为研究组39例接受清热消痈汤内服、消炎散外敷联合抗菌药物治疗,对照组41例接受抗菌药物治疗,治疗1周后观察两组患者降钙素原(procalcitonin,PCT)、证候积分、血清C反应蛋白(C-reaction protein,CRP)水平及中西医疗效与不良反应。结果:治疗后研究组患者肛周疼痛(t=6.941)、便秘(t=9.312)、肛周红肿(t=5.524)、肛门灼热(t=8.065)评分与CRP(t=11.028)、PCT(t=8.588)水平均低于对照组(P<0.05);总有效率研究组为97.44%,高于对照组的87.80%(χ^(2)=6.798,P<0.05);不良反应发生率研究组为17.95%,对照组为14.63%,差异无统计学意义(χ^(2)=0.404,P>0.05)。结论:在抗菌药物治疗的基础上联合清热消痈汤内服及消炎散外敷可有效改善肛周皮下脓肿患者肛门周围炎症,缓解症状且安全性较好。Objective:To observe clinical effects of oral Qingre Xiaoyong Tang(QRXYT)and Xiaoyan powder for external use in the treatment of early perianal abscess and the safety.Methods:Eighty patients suffering perinal subcutaneous abscess were randomized into 39 cases of the study group,who accepted oral QRXYT,Xiaoyan powder for external use and antibacterials,and 41 cases of the control group,who took antibacterials,to observe PCT,syndrome integrals,the levels of CRP,clinical effects of integrative medicine and side effects between both groups after one week of treatment.Results:After the treatment,the study group was lower than the control group in the scores of perianal pain(t=6.941),constipation(t=9.312),perianal redness and swelling(t=5.524)and anal burning(t=8.065),and the levels of CRP(t=11.028)and PCT(t=8.588)(P<0.05);total effective rate of the study group was 97.44%,higher than 87.80% of the control group(χ^(2)=6.798,P<0.05);the incidence of adverse reaction of the study group was 17.95%,lower than 14.63%of the control group,and the difference had no statistical meaning(χ^(2)=0.404,P>0.05).Conclusion:On the foundation of antibacterials,oral QRXYT and Xiaoyan powder for external use could effectively improve perianal inflammation,and relieve the patients'symptoms with better safety.
