摘　　要：目的探讨肺结核患者(Mtb)患者外周血MPT64的相关表达水平的意义。方法选择2020年9月—2022年4月在本院诊断为活动性肺结核的患者40例作为研究对象,并设为Mtb患者组,再细分为活动期组和稳定期组两组,每组各20例;以同期健康体检者40名为对照组。分离外周血中的血清和单个核细胞(PBMC),用实时荧光定量PCR检测PBMC中MPT64 mRNA的表达水平;同时用ELISA法检测血清中MPT64的水平、全血γ干扰素诱导蛋白-10(IP-10)和白介素12(IL-12)水平,并对MPT64、IP-10及IL-12进行比较分析。结果Mtb患者PBMC中MPT64 mRNA表达水平高于对照组(P<0.05),Mtb活动期患者PBMC中MPT64 mRNA表达水平显著高于稳定期患者的表达水平(P<0.05)。Mtb患者血清中MPT64的水平高于对照组(P<0.05),Mtb活动期患者血清中MPT64水平又显著高于稳定期患者(P<0.05)。Mtb患者血清中MPT64的水平与IP-10及IL-12成正相关(r=0.694、0.722,P<0.05)。结论血清MPT64、IP-10及IL-12水平呈现与活动性肺结核病情相关的动态变化,MPT64可作为活动性结核病的辅助诊断方法。Objective To investigate the significance of the expression of MPT64 in peripheral blood of patients with Mycobacterium tuberculosis(Mtb).Methods 40 patients with active Mtb,those were admitted to the hospital from September 2020 to April 2022,were enrolled as study subjects,and they were set as Mtb group.The Mtb patients were divided into active phase group and stable phase group.The control group included 40 healthy subjects during the same period.Serum and peripheral blood mononuclear cells(PBMC)were isolated.The expression of MPT64 mRNA in PBMC was detected by real-time PCR.The levels of MPT64,interferon-γinduced protein-10(IP-10)and interleukin-12(IL-12)in serum were detected by ELISA and the differences of MPT64,IP-10,and IL-12 were compared among groups.Results The expression level of MPT64 mRNA in PBMC of patients with Mtb was significantly higher than that of normal control group(P<0.05).The expression level of MPT64 mRNA in PBMC of patients with active Mtb was significantly higher than that of patients with stable Mtb(P<0.05).The serum level of MPT64 in patients with Mtb was significantly higher than that in normal control group(P<0.05),and the serum level of MPT64 in patients with active Mtb was significantly higher than that in patients with stable Mtb(P<0.05).The level of MPT64 in serum of Mtb patients was positively correlated with IP-10 and IL-12(r=0.694,0.722,P<0.05).Conclusions The serum levels of MPT64,IP-10 and IL-12 showed dynamic changes related to the severity of active tuberculosis.MPT64 could be used as an auxiliary diagnostic method for active tuberculosis.
